摘　　要：目的研究胸腺肽注射液对老年慢性阻塞性肺疾病(COPD)合并肺部感染患者肺功能及免疫功能的影响。方法采用回顾性分析方法,选取2019年1月至2020年6月入院的老年COPD合并肺部感染患者82例,参考不同治疗方式分为研究组和对照组,每组41例。对照组选用抗感染、糖皮质激素及异丙托溴铵雾化吸入治疗,研究组基于对照组选用胸腺肽注射液治疗。比较2组治疗前后肺功能指标(PEF、FVC、FEV1)、免疫功能指标(CD_(3)^(+)、CD_(4)^(+)/CD_(8)^(+)、NK细胞)、血气指标(PaCO_(2)、PaO_(2))水平与治疗后总有效率。结果2组治疗前肺功能指标、免疫功能指标、血气指标水平差异无统计学意义(P>0.05)。2组治疗后较治疗前PEF、FVC、FEV1与PaO_(2)明显更高,PaCO_(2)明显更低,差异均有统计学意义(P<0.05);研究组治疗后较治疗前免疫功能指标水平更高,差异有统计学意义(P<0.05)。研究组较对照组治疗后PEF、FVC、FEV1、CD_(3)^(+)、CD_(4)^(+)/CD_(8)^(+)、NK细胞、PaO2与总有效率明显更高,PaCO_(2)明显更低,差异均有统计学意义(P<0.05)。结论老年COPD合并肺部感染治疗中胸腺肽注射液的应用效果显著,可改善患者血气指标,促使其肺功能、免疫功能快速恢复。Objective To investigate the effects of thymosin injection on pulmonary function and immune function in elderly patients with chronic obstructive pulmonary disease(COPD)complicated by pulmonary infectiont.Methods A total of 82 elderly COPD patients complicated by pulmonary infection who were treated in our hospital from January 2019 to June 2020 were enrolled in the study,who were divided into the research group(n=41)and the control group(n=41)according to different treatment methods.The patients in control group were given anti-infection treatment,glucocorticoid and ipratropium bromide spray inhalation,however,the patients in research group,on the basis of control group,were treated by thymosin injection.The lung function indexes(PEF,FVC,and FEV1),immune function indexes(CD_(3)^(+),CD_(4)^(+)/CD_(8)^(+),NK cells),blood gas indexes(PaCO_(2),PaO)level before and after treatment and total effective rate after treatment were observed and compared between the two groups.Results Before treatment there were no significant differences in the levels of lung function indexes,immune function indexes and blood gas indexes between the two groups(P>0.05).After treatment the levels of PEF,FVC,FEV1 and PaO_(2) in both groups were significantly increased,but PaCO_(2) levels were significantly decreased(P<0.05).Moreover the immune function index levels in research group were superior to those in control group.In addition the levels of PEF,FVC,FEV1,CD_(3)^(+),CD_(4)^(+)/CD_(8)^(+),NK cells,PaO_(2) and the total effective rate in research group were significantly higher than those in control group,but the PaCO_(2) levels were significantly lower than those in control group(P<0.05).Conclusion The application effects of thymosin injection in the treatment of elderly COPD combined with lung infection are significant,which can improve the blood gas index of the patient and promote the rapid recovery of lung function and immune function.
