主动保温技术对经皮肾镜碎石术患者术中低体温和术后寒战的影响

摘　　要：目的观察主动保温技术是否能减少经皮肾镜碎石术患者发生术中低体温和术后寒战。方法选取择期经皮肾镜碎石术患者70例,采用随机数字表将患者分成对照组和主动保温组,每组35例。对照组患者接受常规手术,术中仅以手术毯覆盖行被动保温。主动保温组患者接受加温输液和覆盖充气式保温毯行主动保温,比较两组患者术中低体温和术后寒战的发生率。结果对照组有20例患者发生术中低体温,发生率为57.1%。其中有11例患者发生术后寒战,发生率为31.4%。主动保温组有9例患者发生术中低体温,发生率为25.7%,其中有3例患者发生术后寒战,发生率为8.6%。结论术中联合加温输液和覆盖充气式保温毯行主动保温的技术,可有效减少经皮肾镜碎石术患者术中低体温和术后寒战的发生率。Objective To investigate whether active warming could reduce the incidences of intraoperative hypothermia and postoperative shivering in patients undergoing percutaneous nephrolithotomy.Methods 70 patients undergoing percutaneous nephrolithotomy were randomly assigned to receive either active warming technique or the control treatment,with 35 patients in each.The patients in the control received the surgery as routine,covered with only surgical blanket for passively keeping warm.The patients applied with active warming technique were served with liquid warming and covered with inflatable warming blanket for actively keeping warm.Observations of intraoperative hypothermia and postoperative shivering were recorded.Results There were 20 patients suffering with intraoperative hypothermia in the control,accounting for 57.1%,and there were 11 patients suffering with postoperative shivering,accounting for 31.4%.In contrast,there were 9 patients suffering with intraoperative hypothermia after active warming,with the incidence of 25.7%,and there were only 3 patients suffering with postoperative shivering,with the incidence of 8.6%.Conclusion Combination of liquid warming and inflatable warming blanket for actively keeping warm was able to reduce intraoperative hypothermia and postoperative shivering in patients undergoing percutaneous nephrolithotomy。
