腕踝针联合督灸治疗气虚血瘀型椎动脉型颈椎病临床研究

摘　　要：目的:探讨腕踝针联合督灸治疗气虚血瘀型椎动脉型颈椎病(CSA)的疗效。方法:选取本院针灸科门诊椎动脉型颈椎病患者60例,随机分为治疗组和对照组,各30例。对照组给予普通针刺和督灸干预,治疗组给予腕踝针和督灸干预。比较两组临床疗效,治疗前后颈椎屈伸活动范围、颈椎弧度和椎体水平位移的变化,治疗前后椎-基底动脉收缩期峰值流速(SPV)、平均流速(Vm)、舒张末期流速(EDV)、血流量(Q)、阻力指数(RI)及搏动指数(PI)改善情况。结果:治疗后,治疗组临床疗效优于对照组(P<0.05);治疗组颈椎屈伸活动范围和弧度大、椎体水平位移小,与对照组比较,差异有统计学意义(P<0.05);治疗组SPV、Vm、EDV、Q、RI及PI均改善,与对照组比较,差异有统计学意义(P<0.05)。结论:腕踝针联合督灸能缓解症状,改善脑血管供应,促进颈椎生物力学平衡的恢复。Objective:To explore the effect of wrist-ankle acupuncture combined with Du meridian moxibustion on cervical spondylosis of vertebral artery type(CSA)of qi deficiency and blood stasis.Methods:A total of 60 patients with CSA were randomly divided into the treatment group and the control group,with 30 cases in each group.The control group was treated with ordinary acupuncture and Du meridian moxibustion,whereas the treatment group was treated with wrist-ankle acupuncture combined with Du meridian moxibustion.The clinical curative effects of the two groups were compared.The range of cervical flexion and extension,the curvature of cervical spine,the horizontal displacement of vertebral body,systolicpeak velocity(SPV),average velocity(Vm)and end-diastolic velocity(EDV)of the vertebral-basal artery,as well as the improvements of blood flow(Q),resistance index(RI)and pulsatility index(PI)were compared before and after the treatment in the two groups.Results:The clinical efficacy of the treatment group was better than that of the control group(P<0.05).The cervical spine flexion and extension range and radian were large,and the vertebral body horizontal displacement was small in the treatment group compared to those in the control group after the treatment(P<0.05).SPV,Vm,EDV,Q,RI and PI were all significantly improved in the treatment group compared to those in the control group(P<0.05).Conclusion:Wrist-ankle acupuncture combined with Du meridian moxibustion can relieve symptoms,improve cerebrovascular supply and promote the restoration of cervical spine biomechanical balance.
