沙库巴曲缬沙坦用于老年高血压心脏病的效果观察

摘　　要：目的研究沙库巴曲/缬沙坦(sacubitril/valsartan)治疗老年高血压心脏病患者的临床效果。方法收集2019年5月-2021年1月收入我院老年高血压心脏病患者,总共90例,采用随机表法分为研究组和对照组,每组45例,研究组使用沙库巴曲/缬沙坦,对照组使用缬沙坦,2组持续治疗90 d。观察2组治疗前后血压、左心室收缩末期内径(LVESD)、左室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、血浆脑钠肽(BNP)。结果2组治疗前血压、LVESD、LVEF、LVEDD、BNP无统计学差异(P>0.05),治疗后,研究组血压、LVESD、LVEDD、BNP下降幅度大于对照组(P<0.05),研究组LVEF大于对照组(P<0.05)。研究组的治疗总有效率高于对照组(χ^(2)=6.391,P=0.037),2组不良反应发生率无统计学差异(χ^(2)=0.155,P=0.694)。结论沙库巴曲/缬沙坦治疗老年高血压心脏病效果较好,可降低血压,改善患者心功能。Objective To study the clinical effect of sacubitril/valsartan in the treatment of elderly patients with hy-pertensive heart disease.Method Totally 90 elderly patients with hypertensive heart disease admitted to Shenzhen Bao'an Hos-pital Affiliated to Southern Medical University from May 2019 to January 2021 were selected and randomly divided into ex-perimental group and control group,with 45 cases in each group.The experimental group was treated with sacubitril/valsartan,while the control group was treated with valsartan.The two groups were treated for 90 days.Blood pressure,left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD)and plasma brain natriuretic peptide(BNP)were observed and compared between the two groups before and after treatment.Results There was no significant difference in blood pressure,LVESD,LVEF,LVEDD and BNP between the two groups before treatment(P>0.05).After treatment,the decrease of blood pressure,LVESD,LVEDD and BNP in the experimental group was greater than that in the control group(P<0.05),and the LVEF of the experimental group was greater than that of the control group(P<0.05).The total effective rate of the experimental group was higher than that of the control group(χ^(2)=6.391,P=0.037).There was no significant difference in the incidence of adverse reactions between the two groups(χ^(2)=0.155,P=0.694).Conclusion Sakubatral/valsartan is effective in the treatment of elderly patients with hypertensive heart disease,which can reduce blood pressure and improve cardiac function.
