改良钬激光联合套石篮对复杂性输尿管上段结石的疗效

摘　　要：目的:分析改良钬激光碎石联合套石篮对复杂性输尿管上段结石的疗效。方法:将106例复杂性输尿管上段结石患者按随机数字表分为常规组与改良组,各53例,其中常规组的对象均接受常规钬激光碎石联合套石篮治疗,改良组的对象均接受改良钬激光碎石联合套石篮治疗,改良方法:选取改良的特伦德伦伯格体位,并在输尿管导管旁置持续注水。对比单次碎石成功率、总碎石成功率、结石清除率,碎石时间、术中出血量、总手术时间和总住院费用,术后并发症发生率,随访期间复发率。结果:改良组单次碎石成功率为100.00%,明显高于常规组的88.68%(P<0.05),2组总碎石成功率、结石清除率均相近(P>0.05);改良组碎石时间和总手术时间均短于常规组(P<0.05),改良组术中出血量和总住院费用均少于常规组(P<0.05);改良组尿路感染、总并发症发生率、复发率分别为0.00%、1.89%、0.00%,均低于常规组的11.32%、18.87%、11.32%(P<0.05)。结论:对复杂性输尿管上段结石采用改良钬激光碎石联合套石篮可提高单次碎石成功率,减轻术中创伤,减少总住院费用,控制并发症和复发风险。Objective:To analyze the curative effect of modified holmium laser lithotripsy combined with stone basket for complex upper ureteral calculi.Methods:106 patients with complex upper ureteral calculi were divided into routine group and modified group according to random number table,53 cases in each group.The subjects in routine group were treated with routine holmium laser lithotripsy combined with stone basket insertion.The subjects in improved group were treated with modified holmium laser lithotripsy combined with stone basket insertion.Water.The success rates of single lithotripsy,total lithotripsy success rates,stone clearance rates,lithotripsy time,intraoperative bleeding volume,total operation time and total hospitalization expenses,incidences of complications and recurrence rates during follow-up were compared.Results:The success rate of single lithotripsy in the improved group was 100.00%,which was significantly higher than 88.68%in the routine group(P<0.05).The success rates and stone clearance rates of the two groups were similar(P>0.05).The lithotripsy time and total operation time in the improved group were shorter than those in the routine group(P<0.05),and the bleeding volume and total hospitalization expenses in the improved group were less than those in the routine group(P<0.05).The incidence of urinary tract infection,total complications and recurrence in the improved group were 0.00%,1.89%and 0.00%respectively,which were lower than those of 11.32%,18.87%and 11.32%in the routine group respectively(P<0.05).Conclusion:The improved holmium laser lithotripsy combined with lithotripsy basket can improve the success rate of single lithotripsy,reduce intraoperative trauma,reduce total hospitalization costs,and control the risk of complications and recurrence of complex upper ureteral calculi.
