平板运动试验联合多导联动态心电图对冠心病的诊断价值

摘　　要：目的:研究平板运动试验联合多导联动态心电图对老年人冠心病的诊断意义。方法:选于2017年4月~2019年5月到我院心内科接受治疗的134例疑似冠心病患者,分为TRT组、12LH组、JOINT组,分别对134例疑似冠心病患者进行检测。结果:JOINT组高龄冠心病阳性率(93.26%)均高于TRT组、12LH组、JOINT组阴性率(3.73%)均低于TRT组、12LH组,组间比较有差距,(P<0.05);JOINT组对心肌缺血检出率(94.02%)、心律失常检出率(89.55%)均高于TRT组、12LH组,组间比较有差距,(P<0.05);JOINT组单支病变检测率(60.44%),多支病变检测率(35.82%),均高于TRT组、12LH组,组间比较有差距,(P<0.05);JOINT组ST压低端(85.07%)、MTWA检出率(70.89%)、ST斜率(45.52%)、ST指数(54.47)、ST滞后(76.11)的指标检出率均高于TRT组和12LH组,组间对比有差距(p<0.05);JOINT组敏感度、特异性、ROC分别为80.62%、83.29%、0.831,均高于TRT组和12LH组,组间比较有差距,(P<0.05);结论:平板运动试验联合多导联动态心电图对老年冠心病的诊断准确率高,同时具有经济、无创、安全等诸多优势,对老年冠心病的诊断具有一定的临床指导价值。Objective:To study the diagnostic significance of treadmill exercise test combined with multi-lead Holter in the diagnosis of coronary heart disease in the elderly.Methods:134 patients with suspected coronary heart disease who were treated in the cardiology department of our hospital from April 2017 to May 2019 were divided into TRT group,12LH group,and JOINT group.134 patients with suspected coronary heart disease were tested.Results:The positive rate of coronary heart disease in the elderly in the JOINT group(93.26%)was higher than that in the TRT group,12LH group,and the negative rate(3.73%)of the JOINT group were lower than those in the TRT group and 12LH group.The detection rate of myocardial ischemia(94.02%)and arrhythmia(89.55%)in the JOINT group were higher than those in the TRT group and 12LH group,there was a gap between the groups(P<0.05);single vessel disease in the JOINT group The detection rate(60.44%)and the detection rate of multivessel lesions(35.82%)were higher than those of the TRT group and the 12LH group.There was a gap between the groups(P<0.05);the JOINT group had ST depression(85.07%)and MTWA The detection rate(70.89%),ST slope(45.52%),ST index(54.47)and ST lag(76.11)were higher than those in the TRT group and 12LH group,and there was a gap in the comparison between the groups(p<0.05);The sensitivity,specificity and ROC of JOINT group were 80.62%,83.29%,0.831,respectively,which were higher than TRT group and 12LH group,there was a gap between the groups(P<0.05);Conclusion:Based on treadmill exercise test and 12 lead The specificity of the diagnostic method for senile coronary heart disease is significantly better than that of a diagnostic method.At the same time,it has many advantages such as economy,non-invasiveness,and safety.It has high clinical value and effectively improves the diagnosis rate of clinical diagnosis.
