小柴胡汤联合浮针疗法治疗老年躯体形式疼痛障碍临床评价

摘　　要：目的探讨小柴胡汤联合浮针疗法治疗老年躯体形式疼痛障碍(SPD)的临床疗效。方法选取衡水市精神病医院2018年4月至2019年6月收治的老年SPD患者74例,按随机数字表法分为对照组和研究组,各37例。两组患者均予浮针疗法治疗,研究组患者加用小柴胡汤。根据简化McGill疼痛评分量表(SF-MPQ)减分率评定临床疗效,对比两组患者的疼痛症状,分别采用匹兹堡睡眠质量指数(PSQI)和生活质量综合评定问卷(GQOLI-74)评定治疗前后的睡眠质量和生活质量。观察治疗期间不良反应发生情况。结果研究组总有效率为89.19%,显著高于对照组的70.27%(P<0.05);研究组患者治疗后的McGill疼痛评分量表疼痛评级指数中感觉项、情感项评分及总分,视觉模拟评分,现时疼痛强度评分,PSQI均显著低于对照组(P<0.05);GQOLI-74评分显著高于对照组(P<0.05);对照组与研究组不良反应发生率相当(18.92%比10.81%,P>0.05)。结论小柴胡汤联合浮针疗法治疗老年SPD,可有效缓解疼痛症状,改善睡眠质量和生活质量。Objective To investigate the clinical efficacy of Xiaochaihu Decoction combined with floating needle therapy in the treatment of elderly patients with somatoform pain disorder(SPD).Methods A total of 74 elderly patients with SPD admitted to the Hengshui Psychiatric Hospital from April 2018 to June 2019 were selected and randomly divided into the control group and the study group according to the random number table,37 cases in each group.The patients in the two groups were treated with floating needle therapy,on this basis,the patients in the study group were treated with Xiaochaihu Decoction.The clinical efficacy was evaluated according to the score reduction rate of the Short-Form McGill Pain Questionnaire(SF-MPQ).The pain symptoms of the two groups were compared.The quality of sleep and quality of life before and after treatment were evaluated by the Pittsburgh Sleep Quality Index(PSQI)and the Generic Quality of Life Inventory 74(GQOLI-74)respectively.The adverse reactions during treatment were observed.Results The total effective rate of the study group was 89.19%,which was significantly higher than 70.27%of the control group(P<0.05).After treatment,the sensation score,emotion score and total score in pain rating indexes(PRI)of the SF-MPQ,visual analogue scale(VAS)score,present pain intensity(PPI)score and PSQI in the study group were significantly lower than those in the control group(P<0.05).The GQOLI-74 score in the study group was significantly higher than that in the control group(P<0.05).The incidence of adverse reactions in the control group was similar to that in the study group(18.92%vs.10.81%,P>0.05).Conclusion Xiaochaihu Decoction combined with floating needle therapy in the treatment of elderly patients with SPD can effectively relieve pain symptoms and improve quality of sleep and quality of life.
