重组人碱性成纤维细胞生长因子外用联合氯己定湿敷治疗复发性唇疱疹临床研究

摘　　要：目的探讨重组人碱性成纤维细胞生长因子(rh-b FGF)外用联合氯己定湿敷治疗复发性唇疱疹的临床疗效。方法选取医院2019年1月至2020年3月收治的复发性唇疱疹患者100例,随机分为观察组和对照组,各50例。两组患者均在清创后给予氯己定溶液湿敷,观察组患者在湿敷后外用rh-bFGF治疗。两组均连续治疗1周。结果观察组总有效率为92.00%,显著高于对照组的76.00%(P<0.05);观察组患者疱疹停止出现时间、创面愈合时间、疼痛消失时间、结痂脱落时间均显著短于对照组(P<0.05);观察组患者治疗后的转化生长因子-β1、血管内皮生长因子、b FGF水平均显著高于对照组,C反应蛋白、白细胞介素1β、降钙素原水平均显著低于对照组(P<0.05);观察组与对照组不良反应发生率相当(14.00%比8.00%,P>0.05)。结论rh-b FGF外用联合氯己定湿敷治疗复发性唇疱疹,可缩短创面愈合时间,降低炎性反应水平,升高各生长因子水平,有助于新生血管的形成和肉芽生长。Objective To investigate the clinical efficacy of recombinant human basic fibroblast growth factor(rh-b FGF)for external use combined with chlorhexidine solution wet compress in the treatment of patients with recurrent herpes labialis.Methods A total of 100 patients with recurrent herpes labialis admitted to our hospital from January 2019 to March 2020 were selected and randomly divided into the observation group and the control group,50 cases in each group.The patients in the two groups were given chlorhexidine solution wet compress after debridement,and the patients in the observation group were treated with rh-bFGF after wet compress.Both groups were continuously treated for one week.Results The total effective rate of the observation group was 92.00%,which was significantly higher than 76.00%of the control group(P<0.05).The time of herpes cessation,wound healing,pain disappearance and scab shedding in the observation group were significantly shorter than that in the control group(P<0.05).The levels of transforming growth factor-β1,vascular endothelial growth factor and basic fibroblast growth factor(bFGF)in the observation group were significantly higher than those in the control group,and the levels of C-reactive protein(CRP),interleukin-1β(IL-1β)and procalcitonin(PCT)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(14.00%vs.8.00%,P>0.05).Conclusion rh-bFGF combined with chlorhexidine solution wet compress in the treatment of recurrent herpes labialis can shorten the wound healing time,reduce the level of inflammatory reaction,increase the level of various growth factors,and contribute to the formation of neovascularization and granulation growth.
