摘 要:目的探讨多时期影像学检查中肝脏的变化对急性胰腺炎(AP)患者肝损害的评估价值.方法回顾性分析58例AP患者CT平扫及增强扫描影像资料,其中第1次平扫及增强扫描检查组(AP-1组)58例次,第2次平扫及增强扫描组(AP-2组)17例次,选取35例肝功能正常,影像无肝脏及胰腺病变患者为对照组.分别对3组患者平扫肝脏CT值、增强肝脏CT值,增强肝脏强化率,增强肝脏强化增长值进行对比、统计分析,对比分析其MR表现,并收集整理AP组58例患者行CT检查当日所进行肝功能及血常规检验结果进行匹配分析.结果AP-1组、AP-2组、对照组在平扫肝脏CT值、增强肝脏CT值之间存在统计学差异.AP患者肝功能及血常规异常、MR表现与其肝实质密度减低存在一致性.结论AP患者存在肝实质平扫密度减低且强化减弱,与肝脏血流减缓、肝窦淤积存在一致性,有利于评估AP所致的急性肝损害.Objective To explore the value of multi-stage liver image changes in the assessment of liver damage in patients with acute pancreatitis(AP).Methods The plain and enhanced CT images of 58 patients with AP were analyzed retrospectively.58 patients who received plain and enhanced CT scanning at the first time were assigned to group AP-1,then 17 patients received similar scanning at the second time was group AP-2,meanwhile 35 patients with normal liver function and no damage of liver and pancreas on CT images were collected as the control group.Plain and enhanced liver CT values,liver enhancement rate,liver enhancement increasing value and some MR manifestations of all patients were compared and statistical analyzed.The results of liver function test and blood routine test of 58 patients in AP group at the day of CT examination were collected for matching analysis.Results The AP-1,AP-2 and the control groups had statistical differences in plain and enhanced liver CT value.The decreased density of liver parenchyma in AP patients was consistent with the abnormal liver function,blood routine test and MR manifestations.Conclusion The decreased plain density and enhancement of parenchyma is consistent with the reduction of liver blood flow and hepatic stasis in the patients with AP,which is helpful to evaluate the acute liver damage caused by AP.
