摘 要:目的探讨中高危风险新生儿胆红素血症基底节区T_(1)WI信号的直方图特征.方法回顾性分析49例足月新生儿的MRI和临床资料,包括中高危风险胆红素血症24例,正常胆红素水平25例.患者胆红素检查与MR扫描时间相隔<3d.测量基底节区T1WI信号的直方图参数,对2组病例的观察数据进行独立样本t检验.结果正常对照组和中高危风险胆红素血症组患儿的日龄和矫正日龄等基线特征无显著差异(P=0.430,P=0.058),但2组基底节区T1WI信号的直方图参数的最大信号(Smax)值分别为412.48±78.01和341.13±45.63,平均信号(Smean)值分别为320.33±62.44和268.16±36.33,均有显著性差异(P值均为0.000);Smax/Smean比值分别为1.29±0.04和1.28±0.05,无显著性差异(P=0.471);方差分别为38.42±8.93和30.86±5.45,有统计学差异(P=0.001).Smax和Smean的受试者工作特征(ROC)曲线的曲线下面积(AUC)分别为0.809和0.800,最佳临界点分别为333和261.95.结论基底节区T1WI信号的直方图参数Smax、Smean和方差可以作为新生儿中高危风险高胆红素血症脑损伤早期辅助诊断工具.Objective To investigate the histogram characteristics of T_(1)WI signal in basal ganglia of neonates with medium and high risk bilirubinemia.Methods MRI and clinical data of 49 full-term neonates were analyzed retrospectively,including 24 cases of medium and high risk bilirubinemia and 25 cases of normal bilirubin level.The interval time between bilirubin examination and MR scan was less than 3 days.The signal histogram parameters of bilateral basal ganglia on T_(1)WI were measured,and independent sample t test was performed on the observed data of the two groups.Results There was no significant difference in baseline characteristics such as age and corrected age between the normal control group and the medium and high risk bilirubinemia group(P=0.430,P=0.058).However,the maximum signal(Smax)values of bilateral basal ganglia histogram parameters in the two groups were 412.48±78.01 and 341.13±45.63,and the mean signal(Smean)values were 320.33±62.44 and 268.16±36.33,respectively,with significant differences(P=0.000,P=0.000).The Smax/Smean ratio was 1.29±0.04 and 1.28±0.05.respectively in both groups,with no significant difference(P=0.471).The variances were 38.42±8.93 and 30.86±5.45.respectively,with a significant difference(P=0.001).The area under the curve(AUC)of Smax and Smean were 0.809 and 0.800,and the optimal cut off values were 333 and 261.95,respectively.Conclusion The T_(1)WI signal histogram parameters such as Smax,Smean and variance of bilateral basal ganglia can be used as the early auxiliary diagnostic tools for medium and high risk hyperbilirubinemia encephalopathy.
