胰岛素泵在胰岛素过敏患者脱敏治疗中的应用效果分析

摘　　要：目的观察分析胰岛素泵在胰岛素过敏患者的脱敏治疗效果。方法收集该院2021年1—6月的胰岛素过敏病例5例,应用胰岛素泵进行脱敏治疗,首先对多种胰岛素进行过敏测试,选取过敏最轻的胰岛素进行脱敏治疗,分别配制1%及10%的胰岛素稀释液,胰岛素泵的基础率以0.01 U/h开始,逐渐增加胰岛素泵基础率的浓度,根据患者的反应,加入餐前大剂量输注,最后,皮下注射胰岛素,不出现过敏反应,脱敏成功。结果入组患者脱敏均成功。3例改为门冬胰岛素皮下注射治疗,2例改为赖脯胰岛素皮下注射治疗。5例患者的空腹血糖,餐后2 h血糖均有显著下降,差异有统计学意义(P<0.05)。结论胰岛素泵进行脱敏治疗成功,使既往应用胰岛素过敏的患者可以应用胰岛素治疗控制血糖,血糖控制良好,减少并发症的带来的痛苦。Objective To observe and analyze the desensitization effect of insulin pump in patients with insulin allergy.Methods Collect 5 cases of insulin allergy in the hospital from January to June of 2021,and apply insulin pump for desensitization treatment.Firstly,perform allergy test on a variety of insulins,select the least allergic insulin for desensitization treatment,and prepare 1%and 10%insulin diluent,the basal rate of the insulin pump starts at 0.01 U/h,and gradually increases the concentration of the basal rate of the insulin pump.According to the patient’s response,pre-meal high-dose infusion was added.Finally,insulin was injected subcutaneously without allergies.In response,desensitization was successful.Results All patients were successfully desensitized.3 cases were changed to subcutaneous injection of insulin aspart,and 2 cases were changed to subcutaneous injection of insulin lispro.The fasting blood glucose of 5 patients and the blood glucose of 2 h after meal decreased significantly,and the difference was statistically significant(P<0.05).Conclusion The successful desensitization treatment of the insulin pump allows the patients who have been allergic to insulin to use insulin therapy to control their blood glucose.The blood glucose can be controlled well and the pain caused by complications can be reduced.
