鼻中隔矫正联合下鼻甲成形术治疗伴有鼻中隔偏曲的难治性变应性鼻炎的疗效观察

摘　　要：目的对三线减张鼻中隔矫正联合下鼻甲成形术治疗伴有鼻中隔偏曲的中重度持续性变应性鼻炎方面的有效性进行予以分析观察。方法对难治性变应性鼻炎伴鼻中隔偏曲病人共60例,通过实施三线减张鼻中隔矫正与下鼻甲成形术联用,对比视觉模拟量表(VAS),问卷调查鼻结膜炎生活质量(RQLQ),下鼻甲黏膜嗜酸性粒细胞(EOS)浸润情况及全血中嗜酸性粒细胞绝对值差异,评价术前后1月、术后6月及12月取得效果,对伴鼻中隔偏曲常年性变应性鼻炎病人采用鼻中隔偏曲矫正与下鼻甲成形术后的改善作用。结果与术前比,接受手术的60例患者VAS评分、鼻结膜炎生活质量调查问卷评分均呈大幅度降低(P<0.05),而术后全血及鼻黏膜嗜酸性粒细胞数量下降明显,其差异性较大,存在显著性差异(P<0.01)。结论伴有鼻中隔偏曲的难治性变应性鼻炎人群采用鼻中隔矫正与下鼻甲成形术联用对患者生活质量进行改善,长期疗效有待进一步观察。Objective To observe the curative effects of three minus septoplasty combined with inferior turbinoplasty in the treatment of refractory perennial allergic rhinitis(PAR)patients with spetal deviation.Methods Sixty patients of severe PAR with septal deviation were treated with three minus septoplasty combined with inferior turbinoplasty,compared with visual analogue score(VAS),rhinoconjunctivitis quality of life questionnaire(RQLQ),infiltration of eosinophils in inferior turbinate mucosa and absolute value of eosinophils in blood,respectively before surgery,one month after surgery,six months after surgery and twelve months after surgery follow-up,to observe the change of total effective rate after surgery of refractory perennial allergic rhinitis.Results①With VAS and RQLQ to evaluate the quality of life,the postoperative score decreased significantly,the difference was statistically significant(P<0.05).②The infiltration of eosinophils in inferior turbinate mucosa and absolute value of eosinophils in blood were significantly decreased after surgery,the difference was statistically significant(P<0.01).Conclusion Correction of nasal spetum combined with inferior turbinoplasty can significantly improve the quality of life of patients with refractory perennial allergic rhinitis with spetal deviation,but long-term efficacy needs to be further observed.
