摘 要:目的:研究对比两种造影剂浸泡纱布后作为止血材料在CT图像上伤道显影的效果。方法:以瘦猪肉为研究材料建立模拟伤道,使用造影剂碘比醇和显影比乐浸泡纱布填塞伤道后进行CT成像,并分别设为碘比醇组和显影比乐组,评估两组图像不同区域的灰度差异、伤道分割和三维重建显像效果。结果:模拟伤道中肌肉组织的CT均值为(64.98±7.01)HU,纱布浸泡血液后CT均值为(81.76±21.19)HU,变异系数为35.6;纱布浸泡显影比乐后CT均值为(155.55±28.30)HU,变异系数为23.95;纱布浸泡碘比醇后CT均值为(2705.73±511.83)HU,变异系数为17.61。碘比醇存在硬化伪影;显影比乐呈现蜂窝状纹理。以无填充伤道作为金标,利用阈值分割分别得到显影比乐组与碘比醇组的伤道,Dice相似系数(DSC)分别为0.80和0.74,伤道体积误差分别为17.5%和20.4%。结论:浸泡造影剂的纱布作为止血材料与未浸泡造影剂的普通纱布相比,可以有效区分伤道与周围组织,减少CT分布的离散程度,显影比乐与碘比醇在统计学意义上其分割和重建结果无显著差异,可以为伤道形态学分析和伤情定量评估提供基础。Objective:To study the effects of the image of wound track of two kinds of contrast medias as hemostasis materials in computed tomography(CT)image after they soaked gauzes.Methods:The lean pork was used as object to simulate penetrating wound.The CT image was implemented after the gauzes soaked with contrast medias(Iobitridol and Developing bile)were respectively filled into wound tract.And the Iobitridol group and Developing bile group were respectively designed.And the difference of gray,the segmentation of wound track and the effect of image of three-dimensional reconstruction of different regions of two group were assessed.Results:The CT mean was(64.98±7.01)HU in the muscular tissue of the simulated wound track,and that was(81.76±21.19)HU after gauze soaked with blood,and the coefficient of variation(CV)was 35.6.The CT mean was(155.55±28.30)HU after gauze soaked with Developing bile,and the CV was 23.95.The CT mean was(2705.73±511.83)HU after gauze soaked with Iobitridol,and the CV was 17.61.Harden artifact existed in results of Iobitridol,and Developing bile appeared cellular texture.The wound track without fills was used as golden standard,and threshold segmentation was used to respectively obtain the wound tracks of Iobitridol group and Developing bile group.The Dice similarity coefficients of them were 0.80 and 0.74,respectively.The errors of the volumes of wound tracks were 17.5%and 20.4%,respectively.Conclusion:Compared with common gauze without soaking contrast media,the gauze with soaking contrast media as hemostatic material can effectively distinguish wound track and surrounding tissue,and reduce the dispersion degree of CT distribution.The differences of segmentation and reconstruction between Developing bile and Iobitridol are not significant,and they can provide basis for morphological analysis and quantitative assessment of traumatic condition.
