摘　　要：目的:调查辽宁省9个示范区医疗机构国产医疗器械配置与利用现状以及现有的政策环境,为促进国产医疗器械配置政策创新提供依据。方法:选取示范区的卫计局、医保局以及各级医疗机构的相关负责人及负责医保工作的相关人员共141人进行医疗设备配置现状、当地政府及医疗机构的配置政策、各地医保政策和分级诊疗制度推进情况进行面对面访谈。结果:新农合医保的差异化报销政策及特殊医保政策,一定程度上促进国产医疗器械在医疗机构的配置;家庭医生签约、建立远程医疗系统、云平台和医联体,推动了分级诊疗制度,但普遍存在信息平台利用程度不够;与省内三级甲等医院建立医联体多属于松散型。结论:示范区现行政策环境一定程度上促进了国产医疗器械的配置和利用,未来应加大医疗保险向基层倾斜的杠杆作用,推动国产医疗器械在各级医疗机构配置。Objective:To investigate the status of allocation and utilization of domestic medical devices of medical institutions at 9 demonstration plots in Liaoning Province as well as the existing policy environment,to provide basis for promoting policy innovation of allocation of domestic medical devices.Methods:A total of 141 persons were selected from the relevant responsible persons and relevant personnel who were responsible for health insurance in health and family planning bureaus,health insurance bureaus and the medical organizations of various levels at demonstration plots in Liaoning Province,so as to implement face-to-face interview for the current situation of the configuration of medical equipment,the configuration polices of local government and medical organization,the medical insurance police of various regions and the promoting situation of the diagnosis and treatment system with classification.Results:The differentiated reimbursement policies and special medical insurance policies for the new rural cooperative medical insurance to a certain extent promote the allocation of domestic medical devices in medical institutions;family doctor contract,telemedicine system,cloud platform and health care consortia promoted the hierarchical diagnosis and treatment system,but there was a general lack of utilization;the establishment of health care consortia with tertiary firstclass hospitals in the province was mostly loose.Conclusion:The existing policy environments in all regions promote the allocation and utilization of domestic medical devices to some extent.In the future,the leverage role of the medical insurance in giving priority to the grass-roots level shall be increased to promote the allocation of domestic medical devices in medical institutions at all levels.
