HIV感染者外周血CXCR4/CXCL12表达情况及临床意义

摘　　要：目的观察不同病情状态的HIV感染者外周血趋化因子C-X-C基元受体4(CXCR4)/趋化因子C-X-C基元配体12(CXCL12)的表达情况,并分析其临床意义。方法纳入2017年1月至2019年12月本院收治97例HIV感染者,采用全自动病毒载量仪检测HIV感染者血浆病毒载量,分别依据病毒载量及CD4细胞水平分组;比较患者外周血CXCR4、CXCL12表达情况,分析二者在不同病情HIV感染者中表达意义。结果Ⅱ组(100 copies/mL<病毒载量≤1 000 copies/mL)、Ⅲ组(病毒载量>1 000 copies/mL)的CXCR4表达低于Ⅰ组(病毒载量在40~100 copies/mL),CXCL12表达高于Ⅰ组,且Ⅲ组的CXCR4表达低于Ⅱ组,CXCL12表达高于Ⅱ组(P <0.05);低CD4细胞未治疗组患者CXCR4水平低于高CD4细胞未治疗组,且CXCL12水平高于高CD4细胞未治疗组(P <0.05)。相关性检验结果显示,病毒载量(HIV RNA定量)与HIV感染者外周血CXCR4表达呈负相关(r=-0.856,P <0.05),与CXCL12表达呈正相关(r=0.897,P <0.05);CD4细胞水平与HIV感染者外周血CXCR4表达呈正相关(r=0.796,P <0.05),与CXCL12表达呈负相关(r=-0.871,P <0.05)。结论不同病情状态的HIV感染者外周血CXCR4/CXCL12表达差异显著,CXCR4、CXCL12异常表达可为监测HIV感染者机体状况提供重要信息。Objective To observe chemokine C-X-C motif receptor 4(CXCR4)/chemokine C-X-C motif ligand 12(CXCL12)in peripheral blood of HIV infected patients with different clinical conditions and to analyze clinical significance between the two.Methods 97 HIV infected patients from January 2017 to December 2019 in our hospital were included,using automatic viral load meter to detect plasma viral load of HIV infected patients and grouped them according to viral load and CD4+T cell level.The expressions of CXCR4 and CXCL12 in peripheral blood of patients were compared and the expression significance of the two in HIV infected patients with different conditions was analyzed.Results Comparing the baseline data which including gender,age,infection route,education level,body mass index(BMI)among patients with different viral load groups,no statistically significant differences(P>0.05)were noticed.The expression of CXCR4 and CD4+in groupⅡ(100 copies/mL<viral load≤1000 copies/mL)and groupⅢ(viral load was over 1000 copies/mL)were lower than that of groupⅠ(viral load was 40~100 copies/mL),and the expression of CXCL12 was higher Group I and the presentation of CXCR4 and CD4 cells in groupⅢwere lower than that in groupⅡ,and the manifestation of CXCL12 was higher than that in groupⅡ(P<0.05).The level of CXCR4 in the untreated low CD4 cells group was lower than that in the untreated high CD4 cells group,and the level of CXCL12 was higher than that in the untreated high CD4 cells group(P<0.05).Correlation test results showed that viral load(HIV RNA quantification)was negatively correlated with CXCR4 expression in peripheral blood of HIV-infected patients(r=-0.856,P<0.05)and positively correlated with the expression of CXCL12(r=0.897,P<0.05).The level of CD4+cells was positively correlated with the expression of CXCR4 in peripheral blood of HIVinfected patients(r=0.796,P<0.05)but negatively correlated with the expression of CXCL12(r=-0.871,P<0.05).Conclusion CXCR4/CXCL12 in the peripheral blood of HIV infected patients with di
