凉山州某县艾滋病单阳家庭感染者安全套使用行为影响因素分析

摘　　要：目的探讨凉山州某县单阳家庭HIV/AIDS患者安全套使用行为影响因素,为制订相关干预措施提供依据。方法采用多阶段抽样方法抽取四川省凉山州某艾滋病高流行县符合纳入标准的270名单阳家庭HIV/AIDS患者,采用入户问卷调查,分析单阳家庭HIV/AIDS患者的安全套使用行为及影响因素。结果在270名调查对象中,75.93%(205名)过去半年未坚持使用安全套。多因素Logistic回归分析发现,与安全套使用相关因素包括:拥有生育意愿(OR=0.310,95%CI:0.131~0.733)、同意"使用安全套能阻止把艾滋病传染给配偶"(OR=20.042,95%CI:2.758~159.003)、同意"使用安全套会导致配偶关系变差"(OR=0.412,95%CI:0.181~0.935)、配偶要求使用安全套(OR=5.129,95%CI:2.372~11.091)、在性行为中拥有安全套使用决定权(OR=2.385,95%CI:1.069~5.321)。结论凉山州单阳家庭配偶间存在较高的HIV感染风险,HIV/AIDS患者的生育意愿、对安全套使用的认知以及安全套使用的决定权都会影响单阳家庭中的安全套使用。应针对上述因素给予正确的健康教育和指导,预防单阳家庭艾滋病的配偶间传播。Objective To explore the condom use behaviors and influencing factors of single-positive AIDS patients(referred HIV/AIDS) in Liangshan Prefecture and provide a basis for formulating related intervention measures.Methods A multi-stage stratified sampling method was conducted.A total of 270 AIDS\HIV patients from HIV serodiscordant infected persons who met the inclusion criteria in a high-prevalence county in Liangshan,Sichuan Province were involved.In addition,a household questionnaire survey was used to analyze HIV/AIDS condom use behavior and influencing factors in HIV serodiscordant families.Results Of the 270 respondents,75.93% did not insist on condom use during six months.Multivariate Logistic regression analysis showed that factors influencing condom use included fertility desire(OR=0.310,95%CI:0.131-0.733),agreed that "the use of condoms can prevent the transmission of HIV to the spouse"(OR=20.042,95%CI:2.758-159.003),"the use of condoms will lead to worse spouse relationships "(OR=0.412,95%CI:0.181-0.935),spouse requires condom use(OR=5.129,95%CI:2.372-11.091) and having the rights to decide on condom use.(OR=2.385,95%CI:1.069-5.321) etc.Conclusion This study found a higher risk of HIV infection among HIV serodiscordant families in Liangshan.Among them,the fertility will consist of the infected,the awareness of condom use,and the rights to decide on condom use will affect the condom use in HIV serodiscordant family.In response to these factors,proper health education and guidance,reducing unprotected sex occurs between spouses and preventing the spread of AIDS in HIV sero-discordant families.
