76例艾滋病合并淋巴瘤患者临床特征分析

摘　　要：目的探讨艾滋病合并淋巴瘤患者的临床特征,为规范治疗和临床研究提供参考。方法收集2017年1月至2019年3月云南省传染病医院/云南省艾滋病关爱中心收治的76例艾滋病合并淋巴瘤患者的临床资料,采用回顾性分析方法进行统计分析。结果 76例患者中男女性别比为4.85∶1,平均年龄47.26岁;其中霍奇金淋巴瘤6例,占7.89%;非霍奇金淋巴瘤70例,占92.10%;出现B症状表现50例,占65.79%;合并感染患者42例,占55.26%,细菌性肺炎占17.10%;首发症状为不明原因出现体表无痛性肿物,占59.21%;Ann ArborⅢ、Ⅳ期59例,占77.63%;IPI评分中高、高危(≥3分)42例,占55.26%。结论艾滋病合并淋巴瘤以弥漫大B细胞淋巴瘤为主,其发病隐匿、临床症状缺乏特异性,确诊时常处于疾病晚期,针对疑似病例应早期、全面、动态评估,及时治疗。Objective To investigate AIDS-associated lymphoma patients’ clinical characteristics and provide theoretical basis and data support for standardized treatment and clinical research.Methods The clinical data of 76 patients with AIDS and lymphoma admitted to Yunnan Infectious Disease Hospital/Yunnan AIDS Care Center from January 2017 to March 2019 were collected and retrospectively analyzed.Results Among 76 patients,the sex ratio was4.85∶1,with an average age of 47.26 years.Among them,6 cases(7.89%) were Hodgkin lymphoma and,70 cases(92.10%) were non-Hodgkin lymphoma,50 cases of B symptoms,accounting for 65.79%;42 cases of co-infection,accounting for 55.26%,and bacterial pneumonia accounted for 17.10%;the first symptom was unexplained painless mass,accounting for 59.21%;59 cases of Ann Arbor Ⅲ、Ⅳ,accounting for 77.63%;high IPI score And 42 cases with increased risk(≥3 points),accounting for 55.26%.Conclusion The main types of AIDS complicated with lymphoma were diffuse large B-cell lymphoma,which has hidden onset and lack of specific clinical symptoms.Although it is often diagnosed in the late stage of the disease,suspected cases should be evaluated early,comprehensively,dynamically,and treated in time.
