封闭负压引流技术应用于阴囊坏疽患者中的临床疗效观察

摘　　要：目的:阴囊坏疽患者中应用封闭负压引流技术(vacuum-sealingdrainage,VSD)的临床疗效观察。方法:回顾性分析2010年1月—2020年1月青海红十字医院和华中科技大学同济医学院附属同济医院泌尿外科10年内不同时期收治的阴囊坏疽患者各20例共计40例为研究对象,分成VSD治疗组和常规换药对照组,VSD治疗组采用急诊Ⅰ期下腹部及会阴部坏死性筋膜炎清创术+VSD辅助治疗,同时给予全身联合抗感染及营养支持治疗,待引流液清亮后拆除敷料,创面干净后行二期直接缝合,总结VSD软组织缺损修复与重建的临床疗效。结果:阴囊坏疽患者经早期手术清创+VSD疗法较对照组相比,换药次数减少,住院时间缩短,住院总费用下降,技术操作简便,易于掌握,疗效远优于常规治疗,能够取得用传统方法治疗复杂创面无法达到的良好效果,治疗有显著性差异(P<0.05)。结论:早期创面清创+VSD治疗联合抗生素应用及营养支持治疗阴囊坏疽,能够快速促进伤口的愈合,缩短治疗时间,减少换药次数,降低医疗费用,降低医务人员工作量,值得临床推广应用。Objective:To observe the clinical efficacy of vacuum-sealing drainage(VSD)in patients with scrotal gangrene.Methods:A retrospective analysis of 20cases of scrotal gangrene patients from Qinghai Red Cross Hospital and Tongji Hospital respectively from January 2010to January 2020was performed,and these 40research objects were divided into VSD group and the conventional dressing change group.The VSD treatment group was treated with emergency stageⅠdebridement of lower abdomen and perineal necrotizing fasciitis+VSD adjuvant treatment.At the same time,they were given systemic combined anti-infection and nutritional support treatment.After the drainage fluid was clear,the dressing wound was removed.The wound was cleaned and then directly sutured in the second stage.The clinical effect of VSD technique for soft tissue defect repair and reconstruction were summarized.Results:Compared with the control group,the patients with scrotal gangrene after early surgical debridement+VSD therapy had fewer time of dressing change,shorter hospital stay and lower total hospitalization costs.This technical operation was simple and easy to master.Its curative effect is far superior to conventional treatments,for it can achieve good results in the treatment of complex wounds.The difference was significant(P<0.05).Conclusion:Early wound debridement+VSD therapy combined with antibiotic application and nutritional support for the treatment of scrotal gangrene can quickly promote wound healing,shorten treatment time,reduce the time of dressing change,reduce medical costs and reduce the workload of medical staff,so it is worthy of clinical application.
