顺行和逆行输尿管双J管支架置入术在结核性输尿管狭窄中的疗效对比研究

摘　　要：目的:对比顺行和逆行输尿管双J管支架置入术在结核性输尿管狭窄中的疗效及安全性。方法:选取结核性输尿管狭窄患者81例,试验组39例行顺行输尿管支架管置入术,对照组42例行逆行输尿管支架管置入术,对比两组肾功能指标变化、手术成功率、临床症状缓解率和并发症发生率。结果:试验组手术成功率92.3%高于对照组71.4%,试验组临床症状缓解率82.1%高于对照组59.5%,试验组总并发症发生率25.6%率低于对照组70.7%,试验组肾功能改善优于对照组,两组比较差异均有统计学意义(P<0.05)。结论:在结核性输尿管狭窄的治疗中,顺行输尿管支架置入术比逆行输尿管支架置入具有更高的手术成功率及临床症状缓解率,并发症更少,在临床治疗中值得推广。Objective:To compare the efficacy between retrograde and anterograde double-J ureteral stents in the treatment of tuberculous ureteral stricture.Methods:Eighty-one cases of tuberculous ureteral stricture were selected.The experimental group(n=39)underwent antegrade ureteral stenting,while the control group(n=42)underwent retrograde ureteral stenting.The indexes of renal function,the rate of successful operation,the rate of remission of clinical symptoms and the rate of complications were compared between the two groups.Results:The successful rate of operation was 92.3%in the experimental group and 71.4%in the control group.The clinical symptom remission rate of the experimental group was 82.1%,which was higher than that of the control group(59.5%).The total complication rate of the experimental group was 25.6%,which was lower than that of the control group(70.7%).The improvement of renal function in experimental group was better than that in control group.Conclusion:In the treatment of tuberculous ureteral stricture,antegrade ureteral stent implantation has higher operative success rate and less complications than retrograde ureteral stent implantation has,so it is worth popularizing.
