摘  要:目的探讨特殊部位异位妊娠的磁共振成像(MRI)表现及诊断价值。方法回顾性分析102例经手术病理证实的特殊部位异位妊娠的MRI表现。术前均行MRI常规序列和3D-SPACE序列,其中5例行Gd-DTPA增强扫描。结果102例中,子宫切口瘢痕妊娠72例,子宫宫角妊娠13例(左侧6例,右侧7例);输卵管间质部妊娠10例(左侧4例,右侧6例);输卵管峡部妊娠2例;残角子宫妊娠1例(右残角),宫颈妊娠4例。结论特殊部位异位妊娠既有输卵管妊娠的共性,也有其自身的特殊性。MRI-3D-SPACE结合MRI常规序列能清晰显示孕囊结构、部位,及与周边解剖关系,结合特殊部位异位妊娠的征象特点,有助于特殊部位异位妊娠的早期定位定性诊断,减少异位妊娠的潜在风险。Objective To explore the MRI manifestations and diagnostic value of ectopic pregnancy in special site.Methods The MRI findings of 102 cases of ectopic pregnancy with special site confirmed by operation and pathology were analyzed retrospectively.Routine MRI sequence and 3 D-space sequence were performed before operation,5 of which were enhanced by GD DTPA.Results In 102 cases,72 cases were pregnant with scar of uterine incision,13 cases were pregnant with uterine horn(6 on the left and 7 on the right).There were 10 cases of tubal interstitial pregnancy(4 on the left and 6 on the right),2 cases of isthmus pregnancy,1 case of residual horn pregnancy(right residual horn)and 4 cases of cervical pregnancy.Conclusion Ectopic pregnancy in special site has not only the commonness of tubal pregnancy,but also its own particularity.MRI-3 D-SPACE combined with conventional MRI sequence can clearly show the structure and location of gestational sac,and the anatomical relationship with the surrounding.Combined with the characteristics of ectopic pregnancy in special sites,it is helpful for the early location and qualitative diagnosis of ectopic pregnancy in special sites,and reduces the potential risk of ectopic pregnancy.
