厄贝沙坦氢氯噻嗪联合左旋甲状腺素对老年重症心力衰竭患者B型脑钠肽和sCD40L水平的影响

摘　　要：目的:探讨厄贝沙坦氢氯噻嗪联合左旋甲状腺素对老年重症心力衰竭患者B型脑钠肽(BNP)和可溶性CD40配体(sCD40L)水平的影响。方法:选取2019年10月—2021年2月我院收治的老年重症心力衰竭患者78例作为研究对象,按照随机数字表法将患者分成观察组(n=39)与对照组(n=39)。对照组予以左旋甲状腺素治疗,观察组予以厄贝沙坦氢氯噻嗪联合左旋甲状腺素治疗,比较两组治疗前后BNP、sCD40L、心功能指标及不良反应发生情况。结果:观察组BNP、sCD40L水平低于对照组,差异有统计学意义(P<0.05);观察组LVEF高于对照组,LVDd、LVSd低于对照组,差异有统计学意义(P<0.05);观察组不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论:厄贝沙坦氢氯噻嗪联合左旋甲状腺素治疗可降低老年重症心力衰竭患者B型脑钠肽和sCD40L水平,改善患者心脏功能,且联合用药不增加不良反应发生概率。Objective: To investigate the effect of irbesartan hydrochlorothiazide combined with levothyroxine on the levels of B-type brain natriuretic peptide(BNP) and soluble CD40 ligand(sCD40 L) in the treatment of elderly patients with severe heart failure. Methods: A total of 78 elderly patients with severe heart failure who were admitted in our hospital from October 2019 to February 2021 were selected as the research objects, and the patients were divided into the observation group(n = 39) and the control group(n = 39) according to the random number table method. The control group was treated with levothyroxine, and the observation group was treated with irbesartan hydrochlorothiazide combined with levothyroxine, the BNP, sCD40 L, cardiac function indexes and adverse reactions were compared between the two groups. Results: The levels of BNP and sCD40 L in the observation group were lower than those in the control group, and the difference was statistically significant(P<0.05). LVEF in the observation group was higher than that in the control group, LVDd and LVSd were lower than those in the control group, the difference was statistically significant(P<0.05). The total incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion: Irbesartan hydrochlorothiazide combined with levothyroxine in the treatment of elderly patients with severe heart failure can effectively reduce the levels of B-type brain natriuretic peptide and sCD40 L, improve the cardiac function of patients, and reduce the occurrence of adverse reactions.
