参芪十一味颗粒联合西药治疗肺肾两虚型慢性阻塞性肺疾病稳定期临床研究

摘　　要：目的:观察参芪十一味颗粒联合西药治疗肺肾两虚型慢性阻塞性肺疾病(COPD)稳定期的临床疗效。方法:选取106例肺肾两虚型COPD稳定期患者,根据随机数字表法分为对照组和观察组,每组53例。对照组给予噻托溴铵粉吸入剂、茶碱缓释胶囊治疗,观察组在对照组基础上加用参芪十一味颗粒治疗,2组均治疗3个月。比较2组治疗前后肺功能、免疫功能、BODE指数及血清炎性因子水平。结果:治疗后,2组第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、CD3^(+)、CD4^(+)水平及FEV1/FVC、CD4^(+)/CD8^(+)均较治疗前升高(P<0.05),BODE指数及血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平均较治疗前降低(P<0.05);观察组FEV1、FVC、CD3^(+)、CD4^(+)水平及FEV1/FVC、CD4^(+)/CD8^(+)均高于对照组(P<0.05),BODE指数及血清TNF-α、IL-6、IL-8水平均低于对照组(P<0.05)。结论:参芪十一味颗粒联合西药治疗肺肾两虚型COPD稳定期,可有效改善患者的肺功能及免疫功能,缓解病情,抑制炎性反应。Objective:To observe the clinical effect of Shenqi Shiyiwei granules combined with western medicine for chronic obstructive pulmonary disease(COPD)of lung-kidney deficiency type at stable stage.Methods:A total of 106 cases of COPD patients lung-kidney deficiency type at stable stage were selected and divided into the control group and the observation group according to the random number table method,53 cases in each group.The control group was given tiotropium bromide inhalation powder and theophylline extended-release capsules.The observation group was additionally treated with Shenqi Shiyiwei granules based on the treatment of the control group.Both groups were treated for three months.Before and after treatment,the lung function,immune function,BODE index,and the level of inflammatory factors in serum were compared between the two groups.Results:After treatment,the forced expiratory volume in one second(FEV1),forced vital capacity(FVC),levels of CD3^(+)and CD4^(+),FEV1/FVC,and CD4^(+)/CD8^(+)in the two groups were increased when compared with those before treatment(P<0.05);BODE index,as well as levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and interleukin-8(IL-8)in serum were decreased when compared with those before treatment(P<0.05).Levels of FEV1,FVC,CD3^(+),CD4^(+),as well as FEV1/FVC,and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group(P<0.05);BODE index,as well as levels of TNF-α,IL-6 and IL-8 in serum were lower than those in the control group(P<0.05).Conclusion:In the treatment of COPD of lung-kidney deficiency type at stable stage,Shenqi Shiyiwei granules combined with western medicine can effectively improve the lung function and immune function,relieve the condition of disease,and inhibit inflammatory responses.
