摘 要:目的探讨四步法对比剂注射技术在颈部增强CT检查中的可行性。方法回顾性分析性别、年龄匹配的四步对比剂注射技术(实验组)和常规对比剂注射技术(对照组)的颈部增强CT患者各50例,均右肘静脉注射对比剂并应用相同的扫描参数及重建技术,应用客观和主观评价方法评估并对比分析颈部各血管和非血管结构图像质量。结果在颈部大血管(双侧颈内动脉、颈总动脉、颈内静脉、锁骨下动脉、锁骨下静脉)的主观评价中,实验组各血管充盈均匀度均明显优于对照组;除双侧锁骨下动脉,实验组颈部其余各血管边缘显示均优于对照组;实验组右侧锁骨下静脉周围硬化伪影明显减少,其余各血管周围硬化伪影两组间无显著性差异。在颈部非血管结构(咽喉部、腺体、肌肉、颈部间隙)的主观评价中,两组之间图像质量无显著性差异。在客观评价中,两组间颈部各非血管结构和血管结构的CNR值均无显著性差异。结论四步对比剂注射技术的颈部增强CT可以明显提升对血管结构的显示,具有临床应用可行性。Objective To investigate the feasibility of quadruple-phase contrast material injection in neck-enhanced CT examination. Methods Enhanced neck CT images with quadruple-phase(experimental group) and routine contrast material injection(control group) were retrospectively analyzed, which included 50 gender-and age-matched patients respectively.Image quality was evaluated by objective and subjective methods and compared. Results Compared with control group, contrast material filling in the large cervical vessels were significant more uniformity in experimental group.Except bilateral subclavian arteries, the edge of cervical vessels were more clear in experimental group.The hardening artifacts around the right subclavian vein of the experimental group were significantly reduced in experimental group.However, the hardening artifacts around other vessels were similar between groups.image quality of cervical non-vascular structures were no significant difference between groups by the subjective evaluation.There was no significant difference in image quality by objective evaluation between the two groups. Conclusion The quadruple-phase contrast material injection technique can significantly improve the display of vascular structure in neck enhanced CT,which is feasible for clinical application.
