摘　　要：目的分析左心室同步化参数预测左心室假腱索(LVFT)患者左心室功能下降的价值。方法收集经超声心动图诊断LVFT患者206例的临床资料,采用实时三维超声心动图获得左心室16节段达到最小收缩容积时间的标准差(TMSV16-SD),评估左心室同步性,采用ROC曲线评价其预测左心室功能下降的价值。结果对206例患者随访3年,左心室功能下降组17例,左心室功能正常组189例。左心室功能下降组TMSV16-SD较左心室功能正常组升高[(41.7±9.4) ms vs.(27.7±8.8) ms](P<0.01)。TMSV16-SD预测LVFT患者左心室功能下降的AUC为0.861[95%CI(0.767~0.936)];取截断值为35.25 ms时,其预测LVFT患者左心室功能下降的特异度为76.5%,灵敏度为86.2%,准确度为85.4%。结论左心室同步化参数对LVFT患者左心室功能下降有较好的预测价值。Objective To analyze the value of left ventricular synchronization parameter in predicting the decline of left ventricular function in the patients with left ventricular false tendons(LVFT).Methods The clinical data of 206 LVFT patients diagnosed by echocardiography were collected.The real-time three-dimensional echocardiography was used to obtain the standard deviation of the time to reach the minimum systolic volume in the 16 segments of the left ventricle(TMSV16-SD) to evaluate the left ventricular synchronization.The ROC curve was drawn to evaluate the value in predicting the decline of left ventricular function.Results The patients were followed up for 3 years, during which the left ventricular function was decreased in 17 cases and that was normal in 189 cases.The TMSV16-SD was higher in the patients with decreased left ventricular function than those with normal left ventricular function [(41.7±9.4) ms vs.(27.7±8.8) ms](P<0.01).ROC curve showed that the AUC of TMSV16-SD in predicting the decline of left ventricular function in LVFT patients was 0.861[95%CI(0.767-0.936)].Taking 35.25 ms as the cut-off value, the specificity, sensitivity and accuracy in predicting the decline of left ventricular function in LVFT patients were 76.5%,86.2% and 85.4%,respectively.Conclusion The left ventricular synchronization parameter has good predictive value for the decline of left ventricular function in the patients with LVFT.
