摘 要:目的探讨2型糖尿病(T2DM)并发尿路感染患者血糖、氧化应激及Th细胞亚群水平变化。方法 188例T2DM患者根据是否并发尿路感染分为感染组(74例)和非感染组(114例)。分析感染组患者尿液中病原菌分布情况,监测两组患者FBG、餐后2-h血糖和HbA1c水平,采用ELISA法检测丙二醛(MDA)、超氧化物歧化酶(SOD)和褪黑素(MT)水平,流式细胞术检测Th1细胞、Th2细胞、Th17细胞和调节性T淋巴细胞(Treg细胞)水平。结果感染组共计培养出病原菌81株;其中,革兰阴性菌占83.95%,革兰阳性菌占16.05%。与非感染组相比,感染组FBG、餐后2-h血糖、HbA1c、MDA、Th1、Th17和Treg细胞水平以及Th1/Th2细胞比例均增加,而SOD、MT、Th2细胞水平和Th17/Treg细胞比例降低(P<0.05)。结论 T2DM并发尿路感染患者病原菌以革兰阴性菌为主,他们会出现血糖升高、氧化应激加剧和Th细胞亚群水平失衡等变化。Objective To investigate the changes of blood glucose, oxidative stress and Th cell subsets in the patients with type 2 diabetes mellitus(T2 DM) complicated with urinary tract infection.Methods A total of 188 T2 DM patients were divided into groups of A(with urinary tract infection, 74 cases) and B(without urinary tract infection, 114 cases).The distribution of pathogenic bacteria in the urine was analyzed in group A.The levels of FBG,2-hour postprandial glucose and HbA1 c were monitored.The levels of malondialdehyde(MDA),superoxide dismutase(SOD) and melatonin(MT) were detected by ELISA method.The levels of Th1 cells, Th2 cells, Th17 cells and regulatory T cells(Treg cells) were measured by flow cytometry.Results A total of 81 pathogenic bacterias were cultured in group A,of which Gram-negative bacteria accounted for 83.95% and Gram-positive bacteria taken up 16.05%.Compared with group B,the levels of FBG,2-hour postprandial glucose, HbA1 c, MDA,Th1 cells, Th17 cells, Treg cells and the Th1/Th2 ratio were increased, while the levels of SOD,MT,Th2 cells and the Th17/Treg ratio were decreased in group A(P<0.05).ConclusionThe main pathogens are Gram-negative bacteria in T2 DM patients with urinary tract infection, who are easily complicated with elevated blood glucose, increased oxidative stress and imbalance of Th cell subsets.
