摘  要:目的探讨经皮椎体成形术(Percutaneous vertebroplasty,PVP)治疗骨质疏松性椎体压缩骨折术后形成团块样骨水泥的影响因素。方法回顾性分析自2016-01—2019-06采用PVP治疗的875例骨质疏松性椎体压缩骨折,213例术后出现团块样骨水泥(团块样组),662例术后出现非团块样骨水泥(非团块样组)。比较2组在年龄、性别、病程、是否有外伤史、骨折部位、是否合并骨皮质连续性中断、骨折形态、骨折程度、是否合并椎体裂隙征、骨水泥注入量方面的差异。结果 875例均顺利完成手术,单因素分析结果显示团块样组与非团块样组在年龄、病程、是否合并骨皮质连续性中断、骨折类型、骨折程度、是否合并椎体裂隙征、骨水泥注入量方面比较差异有统计学意义(P<0.05)。多因素Logistic回归分析显示病程较长、骨折程度重、合并椎体裂隙征、骨水泥注入量较多是骨质疏松性椎体压缩骨折PVP术后合并团块样骨水泥的危险因素(P<0.05)。结论病程较长、骨折程度重、合并椎体裂隙征、骨水泥注入量较多是骨质疏松性椎体压缩骨折PVP术后出现团块样骨水泥的危险因素。Objective To investigate the predictors of solid type of cement distribution in percutaneous vertebroplasty (PVP)for osteoporotic vertebral compression fractures.MethodsA total of 875 osteoporotic vertebral compression fracture patients who underwent PVP between January 2016 and June 2019 were retrospectively reviewed,213 cases were identified postoperatively to be solid type in term of bone cement distribution (solid type group),and 662 cases were founded to be non-solid type(non-solid type group).The two groups were compared in terms of age,gender,duration of symptoms,trauma history,fracture site,cortical disruption,fracture type,fracture severity,cleft sign,cement volume.ResultsAll the 875 cases were operated successfully.The results of univariate analysis showed that there were statistical differences between the solid type group and the non-solid type group in terms of age,duration of symptoms,cortical disruption,fracture type,fracture severity,cleft sign,cement volume (P<0.05).Multivariate analysis showed that significant predictors related to solid type of cement distribution were longer duration of symptoms,higher grade of fracture severity,cleft sign and greater cement volume (P<0.05).Conclusion Longer duration of symptoms,higher grade of fracture severity,cleft sign and greater cement volume are significantly associated with solid type of cement distribution in PVP for osteoporotic vertebral compression fractures.
