单纯撑开弹性棒内固定治疗单节段轻中度腰椎间盘突出症的疗效观察

摘　　要：目的分析单纯撑开弹性棒内固定治疗单节段轻中度腰椎间盘突出症的临床疗效。方法回顾性分析自2017-01—2018-12采用单纯撑开弹性棒内固定治疗的48例单节段轻中度腰椎间盘突出症,比较术前与末次随访时的疼痛VAS评分、JOA评分、椎间隙高度、椎间孔高度、腰椎总活动度。结果 48例均顺利完成手术并获得至少6个月的随访,未出现切口感染、血肿、下肢深静脉血栓、内固定物松动、移位等并发症。末次随访时疼痛VAS评分较术前降低,JOA评分、椎间隙高度、椎间孔高度较术前增加,差异有统计学意义(P<0.05);术前与末次随访时的腰椎总活动度比较差异无统计学意义(P>0.05)。结论单纯撑开弹性棒内固定治疗单节段轻中度腰椎间盘突出症可取得满意的疗效,值得在临床推广。Objective To analyze the clinical effect of simple intervertebral space stretching and elastic rod internal fixation in the treatment of mild to moderate single-segment lumbar disc herniation.MethodsForty-eight patients with mild to moderate single-segment lumbar disc herniation treated with simple intervertebral space stretching and elastic rod internal fixation from January 2017 to December 2018 were analyzed retrospectively. The pain VAS score, JOA score, intervertebral space height, intervertebral foramen height and total range of motion of lumbar vertebrae were compared before operation and at the last followup.ResultsAll the 48 cases completed the operation successfully and were followed up for at least 6 months. There were no complications such as incision infection, hematoma, lower limb deep venous thrombosis, internal fixation loosening, displacement. At the last follow-up, the VAS score of pain decreased, while the JOA score, the height of intervertebral space and the height of intervertebral foramen increased compared with those before operation, and the difference was statistically significant(P<0.05). There was no statistically significant difference in the total range of motion of the lumbar spine before operation and at the last follow-up(P>0.05).ConclusionSimple intervertebral space stretching and elastic rod internal fixation can achieve satisfactory results in the treatment of single-segment mild to moderate single-segment lumbar disc herniation, which is worth popularizing in clinical practice.
