活血止痛系列药物在骨伤科领域应用现状的文献研究

摘　　要：目的:探讨活血止痛系列药物在骨伤科领域的应用现状。方法:应用计算机检索中国知网、万方数据库、维普网、中国生物医学文献数据库、PubMed、Cochrane Library,搜集应用活血止痛系列药物(药物组成为当归、三七、醋乳香、土鳖虫、煅自然铜、冰片,剂型不限)治疗骨伤科疾病的相关文献,检索时限均为建库至2019年9月。同时追溯纳入文献的参考文献,以补充获取相关文献。经过文献筛选、数据提取后,总结分析活血止痛系列药物治疗的骨伤科疾病类型、临床疗效及安全性。结果:共纳入31篇文献,其中随机对照试验文献22篇、非随机对照试验文献2篇、病例系列研究文献5篇、病例报告文献1篇、队列研究文献1篇;涉及的活血止痛系列药物包括活血止痛软胶囊和活血止痛胶囊2种,其中有关活血止痛软胶囊的文献15篇、有关活血止痛胶囊的文献15篇、二者比较的文献1篇。31篇文献涉及的疾病主要为急/慢性软组织损伤(8篇)、膝骨关节炎(6篇)、腰背/腿痛(5篇),其中活血止痛胶囊治疗的疾病类型更多,还被用于治疗肩周炎和神经根型颈椎病。活血止痛软胶囊和活血止痛胶囊在缓解各种骨伤科疾病引起的疼痛和肿胀方面均有较好的疗效,其中前者的效果更好。入选文献报道的活血止痛软胶囊和活血止痛胶囊的不良反应大部分较为轻微。结论:目前临床上用于治疗骨伤科疾病的活血止痛系列药物以活血止痛胶囊和活血止痛软胶囊为主,主要被用于治疗急/慢性软组织损伤、膝骨关节炎、腰背/腿痛;2种药物在缓解各种骨伤科疾病导致的疼痛和肿胀方面均有较好的疗效,且安全性较高;活血止痛胶囊治疗的疾病类型更多,但活血止痛软胶囊缓解疼痛和肿胀的效果更好。Objective:To explore the application status of drugs for Huoxue Zhitong(活血止痛,HXZT)in orthopedics and traumatology.Methods:The articles about HXZT drugs(ingredients:Angelica sinensis,Panax notoginseng,Vinegar-processed Olibanum,Eupolyphaga Steleophaga,Calcined pyritum and Borneolum Syntheticum in any dosage forms)for treatment of orthopedic traumatologic diseases included from database establishing to September 2019 were retrieved from China national knowledge internet,WanFang Database,Vip Database,Chinese biomedical literature database,PubMed and Cochrane Library through computer.The references to the included articles were traced for acquiring and supplementing the relevant articles.After articles screening and data extraction,the types of orthopedic traumatologic diseases that clould be treated by HXZT drugs,and their clinical curative effects and safety were summarized and analyzed.Results:Thirty-one articles(involving HXZT soft capsules and HXZT capsules)were included in the final analysis,including 22 randomized controlled trial(RCT)articles,2 non-RCT articles,5 case-series study articles,1 case report article and 1 cohort study article,among which 15 articles for HXZT soft capsules,15 articles for HXZT capsules and 1 for comparison between HXZT soft capsules and HXZT capsules.The diseases mainly included acute/chronic soft tissue injury(8),knee osteoarthritis(6)and low back/leg pain(5)in the 31 articles,and more diseases such as shoulder periarthritis and cervical spondylotic radiculopathy could be treated by HXZT capsules.Both HXZT soft capsules and HXZT capsules had good curative effects in relieving pain and swelling resulting from orthopedic traumatologic diseases,and the former worked better compared to the latter.Most of the adverse reactions caused by HXZT soft capsules and HXZT capsules were mild in the included articles.Conclusion:At present,the HXZT drugs used for treatment of orthopedic traumatologic diseases in clinic consist mainly of HXZT capsules and HXZT soft capsules,which are used
