摘 要:目的研究抑郁症患者的动态功能脑网络以及图论属性异常。方法选取2017年6月至2018年6月就诊于甘肃省天水市第三人民医院的51例抑郁症患者(抑郁症组)以及同期招募的61名年龄、性别相匹配的健康对照者(对照组)进行静息态功能磁共振成像数据采集,采用17项汉密尔顿抑郁量表(17-Item Hamilton Depression Scale,HAMD17)及汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)对患者组的症状程度进行评估。使用组独立成分分析、滑动窗分析以及图论方法探究抑郁症患者动态功能连接与网络拓扑可变性的异常。结果提取了4个重复出现的瞬时功能连接状态,其中状态二和状态四的时间属性以及动态功能连接的组间差异具有统计学意义:(1)抑郁症组在状态二中的时间分数(Z=-2.653,P=0.008)与平均驻留时间(Z=-2.419,P=0.016)显著高于对照组。(2)抑郁症组在状态四中的时间分数(Z=2.470,P=0.014)与平均驻留时间(Z=2.240,P=0.025)显著低于对照组。(3)抑郁症组在状态二中表现出默认模式网络与认知控制网络间的连接显著减弱,而小脑网络与视觉网络间的连接显著增强。(4)在状态四中,抑郁症组功能连接的异常改变主要表现在认知相关网络与感知相关网络间连接减弱及皮质下网络与大脑皮质连接增强。(5)抑郁症组呈现出较低的全局效率变异性(Z=2.036,P=0.042)。结论抑郁症患者动态功能连接、时间属性在不同状态下均发生了改变,同时全局效率可变性降低。Objective To explore the abnormality in dynamic functional brain network and graph properties of patients with major depressive disorder(MDD).Methods In this study,51 MDD patients with depression who were admitted to the Third People′s Hospital of Tianshui City,Gansu Province from June 2017 to June 2018,and 61 healthy controls(HC)with age and gender matched were recruited.The resting state functional magnetic resonance imaging(fMRI)data were collected,and the 17-Item Hamilton Depression Scale(HAMD17)and Hamilton Anxiety Scale(HAMA)were used to evaluate their depression.By using group independent component analysis to resting-state fMRI data,the sliding windows approach and K-means clustering,the dynamic functional network connectivity(FC)and network topology variability were explored.Results Four recurring transient dynamic functional states were identified,among which the State2 and State4 exhibited significant group differences in FC and temporal properties.(1)Compared with HCs,MDD patients exhibited longer fraction time(Z=-2.653,P=0.008)and mean dwell time(Z=-2.419,P=0.016)in State2.(2)On the contrary,the MDD patients presented with a shorter fraction time(Z=2.470,P=0.014)and mean dwell time(Z=2.240,P=0.025)in State4.(3)In State2,MDD patients showed that the connection between the default mode network and the cognitive control network was significantly weakened,while the connection between the cerebellar network and the visual network was significantly enhanced.(4)In state 4,the abnormal changes in FC of the depression group were mainly manifested in the weakening of the connection between the cognitive-related network and the perception-related network,and in the strengthening of the connection between the subcortical network and the cerebral cortex.(5)Further the exploration on graph-theory analysis suggested that MDD patients had lower global efficiency variability(Z=2.036,P=0.042).Conclusion The dynamic FC and temporal properties of MDD patients have changed in two different states,and the variability of
