摘 要:目的比较4种减少腰椎磁共振T1WI快速自旋回波序列血管博动伪影方法的临床应用。方法随机收集35例在协和医院行腰椎MRI平扫加增强检查的患者,每位患者均行4种方法扫描:A组行常规腰椎矢状位扫描,平行于人体正中矢状面,并施加饱和脉冲;B组在A组扫描方法的基础上,使饱和带紧贴椎体前缘;C组在A组扫描方法基础上施加层面方向流动补偿;D组在矢状位上倾斜视野,使之平行于腹主动脉长轴走行进行腰椎矢状位扫描,施加饱和脉冲,同时施加层面方向流动补偿。分别记录A、B、C、D各组的信号强度(SI),标准差(N),并计算信噪比(SNR),测量最大层面伪影面积。结果A、B、C、D组主观评分分别为(3.80±0.70)、(4.40±0.49)、(4.54±0.50)、(4.97±0.16)分;平均伪影面积分别为(16.78±7.98)、(9.07±6.36)、(8.91±6.35)、(1.35±3.01)cm^(2);D组主观评分高、平均伪影面积小,组间比较差异均有统计学意义(均P<0.01);各组信噪比(SNR)差异无统计学意义(均P>0.05)。结论腰椎MR增强后扫描矢状位T1WI快速自旋回波序列时,在矢状位上倾斜视野,使之平行于腹主动脉长轴走行,能有效地减少血管搏动伪影对腰椎图像质量的影响,满足临床诊断的需要。Objective To compare 4 methods for reducing vessel motion artifact of lumbar using magnetic resonance T1 WI fast spin-echo sequences.Methods Totally,35 patients with lumbar MRI plain scan were collected,and enhanced examination was performed.The patients were divided into 4 groups according to 4 scanning methods:group A,traditional lumbar sagittal scanning,parallel to the median sagittal plane,and saturated pulse was applied;group B,based on methods in group A,the saturation band was close to the anterior edge of the vertebral body;group C,flow compensation was applied(slice level direction)based on methods in group A;group D,in sagittal tilt field of vision,parallel to the long axis of the abdominal aorta sagittal scanning,saturation pulse was applied in addition to flow compensation(slice level direction).Signal intensity(SI)standard deviation(N),and the SNR(SNR),quantitative measurement of maximum level artifact area were recorded.Results Artifacts of lumbar vertebrae in group B decreased significantly.Subjective score in groups A,B,C and D was(3.80±0.70),(4.40±0.49),(4.54±0.50)and(4.97±0.16),respectively;average artifact area was(16.78±7.98)、(9.07±6.36)、(8.91±6.35)and(1.35±3.01)cm^(2).The subjective score of group D was higher,the average artifact area was smaller,with significant differences between the groups(P<0.01).There was no significant difference in signal-to-noise ratio(SNR)between the groups(P>0.05).Conclusion When the sagittal T1 WI fast spin echo sequence was used after lumbar MR enhancement,the field of view is tilted in the sagittal position so that it runs parallel to the long axis of the abdominal aorta,the effect of vascular pulsation artifacts on the lumbar image quality can be effectively reduced to meet the needs of clinical diagnosis.
