摘  要:目的 研究脂浊对免疫透射比浊法测定血清前白蛋白等6个项目的干扰。方法 按照NCCLS推荐的EP7-A2和EP9-A2方案,选取40份无脂浊、无溶血、无黄疸患者血清样本,分别以低速离心(3 500 r/min×7 min)与高速离心(16 000 r/min×10 min)测定血清前白蛋白(PA)、脂蛋白a[Lp(a)]、载脂蛋白A1(Apo AI)、载脂蛋白B(Apo B)、胱抑素C(Cys-C)、超敏C-反应蛋白(hs-CRP);再选取40份不同脂浊指数(脂浊指数分别为+、++、+++和++++各10份)患者血清样本,分别以高速与低速离心获得血清,低速离心后的血清分为3份,其中一份不做处理、另外2份分别用生理盐水进行2倍稀释与3倍稀释后与高速离心后的血清同时测定PA、Lp(a)、Apo AI、Apo B、Cys-C、hs-CRP 6个项目。结果 无脂浊血清标本PA、Lp(a)、Apo AI、Apo B、Cys-C、hs-CRP低速与高速离心检测结果差异无统计学意义(P>0. 05);不同程度脂浊标本低速离心后Lp(a)、Apo AI、Apo B、Cys-C、hs-CRP检测结果与高速离心后的检测结果均无明显偏倚;脂浊指数<55. 8时,对PA检测无明显干扰,当脂浊指数≥55. 8时,低速离心检测PA存在明显负干扰;脂浊指数≥63. 9时,标本二倍稀释PA还存在负干扰;脂浊指数≥93. 3时,标本3倍稀释PA也还存在负干扰。结论 脂浊对免疫透射比浊法测定血清Lp(a)、Apo AI、Apo B、Cys-C、hs-CRP无明显干扰;轻度脂浊对透射比浊法测定PA无明显干扰,当脂浊指数≥55. 8时,对PA测定存在明显负干扰,需经高速离心后重新检测或标本稀释后再检测。Objective To study the interference of lipid turbidity to the determination of 6 items of serum prealbumin by immunity transmission turbidity.Methods According to EP7-A2 and EP9-A2 recommended by NCCLS,forty serum samples from patients without lipid turbidity,hemolysis and jaundice were collected.Prealbumin(PA),lipoprotein a[Lp(a)],Apo AI,Apo B,Cystatin C(Cys-C)and hypersensitive C-reactive protein(hs-CRP)were measured by low—speed centrifugation(3500 r/min,7 min)and high-speed centrifugation(1600()r/min,10 min)respectively.Then 40 serum samples from patients with different turbidity indices(respectively+,++,+++and++++,10 samples each)were selected.Serum samples were obtained by centrifugation at high speed and low speed,respectively.The serum samples after low speed centrifugation were divided into three parts.One of the samples was not treated,and the other two samples were treated with normal saline for 2 times dilution and 3 times dilution and high speed centrifugation for simultaneous determination of PA,Lp(a),Apo AI,Apo B,Cys—C and hs—CRP.Results There was no statistical significance on the difference between low speed and high speed centrifugal detection results of PA,Lp(a),Apo AI,Apo B,Cys-C and hs-CRP in serum samples without turbidity(P>0.05).There was no obvious bias between Lp(a),Apo AI,Apo B,Cys-C and hs-CRP detection results after low speed centrifugation and high speed centrifugation.When the turbidity index was less than 55.8,there was no obvious interference to PA detection.When the turbidity index was^=55.8,there was significant negative interference to PA detection.When the turbidity index was M 63.9,there was negative interference in the double dilution of PA.When the turbidity index was greater than or equal to 93.3,there was also negative interference in triple dilution of PA.Conclusion Lipid turbidity had no significant interference to the determination of serum Lp(a),Apo Al,Apo B,Cys-C and hs-CRP by immunity transmission turbidity.Mild turbidity had no significant interference to the
