复方黄硝敷袋治疗腹部外科手术后胃肠功能障碍临床观察

摘　　要：目的:观察复方黄硝敷袋治疗腹部外科手术后胃肠功能障碍的临床疗效。方法:行腹部外科术患者196例随机分为两组各98例。对照组在常规处理基础上采用腹带加压包扎固定。观察组在对照组治疗基础上再加复方黄硝敷袋外敷治疗。观察两组患者治疗前后腹内压(IAP)、中医症状积分、胃肠激素水平等指标的变化,比较两组疗效和术后并发症发生率。结果:观察组总有效率为96.94%,明显高于对照组的81.63%(P<0.01)。治疗后,两组患者的平均腹内压及其分级、中医症状评分、血清胃泌素水平等指标均较治疗前明显降低(P<0.05或P<0.01),血浆胃动素、生长抑素则较前明显升高(P<0.05);且观察组各项指标均优于对照组(P<0.05)。观察组术后并发症发生率显著低于对照组(P<0.01)。结论:腹部外科术后患者使用复方黄硝敷袋外敷治疗可显著改善患者中医症状,调节胃肠激素分泌,加快术后胃肠功能恢复,安全有效。Objective:To evaluate the clinical efficacy of Fufang Huangxiao dressing in the treatment of gastrointestinal dysfunction after abdominal surgery.Methods:Totally 196 patients undergoing abdominal surgery in our hospital from January 2018 to December 2019 were divided into two groups according to the random number table method with 98 cases in each group.The control group was fixed by compression bandaging with abdominal bands on the basis of the conventional treatment.On this basis,the observation group was additionally treated with Fufang Huangxiao dressing for external application.The intra-abdominal pressure,TCM symptom score,gastrointestinal hormone levels and postoperative complications were compared between the two groups.Results:The total effective rate of the observation group was 96.94%,which was higher than that(81.63%)of the control group,and the difference was statistically significant(P<0.01).After treatment,the average intra-abdominal pressure,TCM symptom score and level of Gas in the two groups were significantly reduced with significant differences when compared with those before treatment(P<0.05 or P<0.01).The levels of MTL and SS in the two groups were significantly increased with significant differences when compared with those before treatment(P<0.05).The reduction and the increase in the observation group were more significant than those in the control group with significant differences(P<0.05).The postoperative morbidity rate in the observation group was lower than that in the control group(P<0.05).Conclusion:The use of Fufang Huangxiao dressing for external application to patients after abdominal surgery can significantly improve the symptoms of traditional Chinese medicine in patients,regulate gastrointestinal hormone secretion and accelerate the recovery of gastrointestinal function after surgery,indicating the safety and effectiveness of the preparation.
