利伐沙班与传统抗凝治疗肺栓塞非高危组的临床有效性研究

摘　　要：目的:分析利伐沙班与传统抗凝治疗肺栓塞非高危组的临床有效性。方法:2019年4月-2020年4月收治非大面积急性肺栓塞循环稳定患者92例,依照使用抗凝药物不同分为两组。观察组采用低分子肝素+利伐沙班治疗;对照组采用低分子肝素+华法林治疗。比较两组治疗效果。结果:两组治疗总有效率比较,差异无统计学意义(P>0.05)。观察组不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。两组治疗后心率、呼吸频率及D-二聚体水平均低于治疗前,PaCO_(2)、PaO_(2)水平均高于治疗前,差异有统计学意义(P<0.05);两组治疗后各项指标水平比较,差异无统计学意义(P>0.05)。结论:针对急性非大面积肺栓塞患者的治疗,以低分子肝素为基准,应用利伐沙班治疗的效果满意,不良反应发生率低。Objective:To explore the clinical efficacy of rivaroxaban and traditional anticoagulation in the treatment of non high risk group of pulmonary embolism.Methods:From April 2019 to April 2020,92 patients with stable circulation of non large area acute pulmonary embolism were selected.They were divided into two groups according to the use of anticoagulants.The observation group was treated with low molecular weight heparin plus rivaroxaban.The control group was treated with low molecular weight heparin plus warfarin.We compared the therapeutic effects of the two groups.Results:There was no significant difference in the total effective rate between the two groups(P>0.05).The incidence of adverse reactions in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).After treatment,the heart rate,respiratory rate and D-dimer levels of the two groups were lower than those before treatment,and the levels of PaCO_(2)and PaO_(2)were higher than those before treatment,the difference was statistically significant(P<0.05).There was no significant difference in various index levels after treatment between the two groups(P>.05).Conclusion:For the treatment of patients with acute non large area pulmonary embolism,the effect of rivaroxaban based on low molecular weight heparin is satisfactory,and the incidence of adverse reactions is low.
