摘  要:目的分析血清降钙素原、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、可溶性肿瘤坏死因子受体(sTNFR-Ⅱ)在新生儿败血症中的表达水平及其临床意义。方法抽取2018年2月至2021年2月驻马店市中医院收治的新生儿败血症患儿106例为败血症组,并以1∶1配比抽取同期医院无任何感染症状的106例健康新生儿作为对照组。比较两组血清因子水平,败血症组不同危重程度患儿血清因子水平及败血症组治疗前后血清因子水平。分析血清因子水平和新生儿败血症危重程度的相关性。结果败血症组血清降钙素原、TNF-α、CRP、sTNFR-Ⅱ水平高于对照组,差异有统计学意义(P<0.05)。败血症组不同危重程度患儿血清降钙素原、TNF-α、CRP、sTNFR-Ⅱ水平比较差异有统计学意义(P<0.05)。败血症组极危重患儿血清降钙素原、TNF-α、CRP、sTNFR-Ⅱ水平高于非危重和危重患儿(P<0.05)。败血症组治疗后血清降钙素原、TNF-α、CRP、sTNFR-Ⅱ水平低于治疗前,差异有统计学意义(P<0.05)。Pearson分析结果显示,降钙素原(r=0.649)、TNF-α(r=0.705)、CRP(r=0.681)、sTNFR-Ⅱ(r=0.615)和新生儿败血症危重程度呈正相关(P<0.05)。结论新生儿败血症患儿血清降钙素原、TNF-α、CRP、sTNFR-Ⅱ水平异常升高,与病情危重程度呈正相关,并与治疗效果具有相关性,临床应及时监测血清降钙素原、TNF-α、CRP、sTNFR-Ⅱ水平,为疾病评估提供信息支持,指导临床制定最佳治疗方案。Objective To analyze the expression and clinical significance of serum procalcitonin(PCT),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),and soluble tumor necrosis factor receptorⅡ(sTNFR-Ⅱ)in neonatal sepsis.Methods A total of 106 children with neonatal sepsis treated in Traditional Chinese Medicine Hospital of Zhumadian from February 2018 to February 2021 were selected as the sepsis group,and 106 healthy newborns without any symptoms of infection in the hospital during the same period were selected as the control group,according to a 1∶1 ratio principle with sepsis group.The levels of serum factors were compared between the two groups,and among neonates with different degrees of sepsis in the sepsis group.And preoperative and postoperative levels of serum factors of sepsis group were compared.The correlation between serum factor levels and severity of neonatal sepsis was analyzed.Results The levels of serum PCT,TNF-α,CRP and sTNFR-Ⅱin the sepsis group were higher than those in the control group,and the difference was statistically significant(P<0.05).There were significant differences in serum PCT,TNF-α,CRP,sTNFR-Ⅱlevels among neonates with different disease degrees in the sepsis group(P<0.05).The levels of serum PCT,TNF-α,CRP,and sTNFR-Ⅱof extremely critical neonates were higher than those of non-critical and critical neonates(P<0.05).Postoperative levels of serum PCT,TNF-α,CRP,and sTNFR-Ⅱwere lower than preoperative levels in the sepsis group,and the difference was statistically significant(P<0.05).Pearson analysis showed that PCT(r=0.649),TNF-α(r=0.705),CRP(r=0.681),sTNFR-Ⅱ(r=0.615)were positively correlated to severity of neonatal sepsis(P<0.05).Conclusions The expressions of serum PCT,TNF-α,CRP,and sTNFR-Ⅱin children with neonatal sepsis are abnormally elevated,which is positively correlated to severity of the disease and treatment effect.In clinical,serum PCT,TNF-α,CRP,and sTNFR-Ⅱlevels should be monitored in time to provide information support for disease assessment an
