楂芪降糖汤联合穴位埋线治疗糖尿病肾病的临床观察

摘　　要：目的探讨楂芪降糖汤联合穴位埋线治疗糖尿病肾病(DN)的临床效果。方法抽取2019年1月至2020年12月驻马店市第一人民医院收治的94例气阴两虚血瘀证DN患者作为研究对象,经随机数字表法分为观察组47例和对照组47例。对照组实施常规治疗方案,观察组实施常规治疗+楂芪降糖汤联合穴位埋线治疗方案,两组均连续治疗12周。治疗前、治疗12周后,评估两组中医证候评分、血糖、肾功能,统计并比较两组治疗期间不良反应发生情况。结果治疗12周后,两组咽干口燥、倦怠乏力评分均降低,且观察组低于对照组(P<0.05);两组患者糖化血红蛋白(HbA1c)、空腹血糖(FBG)、餐后2 h血糖(2hPBG)降低,且观察组低于对照组(P<0.05);两组肌酐(SCr)、尿素氮(BUN)、尿白蛋白排泄率(UAER)均降低,且观察组低于对照组(P<0.05);两组不良反应比较,差异未见统计学意义(P>0.05)。结论楂芪降糖汤联合穴位埋线治疗DN利于减轻症状,改善血糖、肾功能指标,且不增加不良反应,安全性较高。Objective To analyze the clinical effect of zhaqi jiangtang decoction combined with catgut implantation at acupoint in the treatment of diabetic nephropathy(DN).Methods A total of 94 DN patients with qi and yin deficiency and blood stasis syndrome in the First People’s Hospital of Zhumadian from January 2019 to December 2020 were selected as the research subjects,They were divided into observation group(47 cases)and control group(47 cases)by a random number table method.The control group was given conventional treatment regimen,and the observation group was given treatment regimen of conventional treatment+zhaqi jiangtang decoction combined with catgut implantation at acupoint,both groups were treated continuously for 12 weeks.Before and 12 weeks after treatment,the TCM syndrome score,blood glucose,renal function between the two groups were evaluated,the occurrence of adverse reactions during treatment in the two groups were compared.Results After 12 weeks of treatment,the scores of throat dryness and fatigue in both groups were decreased,and those in the observation group was lower tan those in the control group(P<0.05);glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),2 hours postprandial blood glucose(2hPBG)of patients in both groups were decreased,and those in the observation group were lower than those in the control group(P<0.05);serum creatinine(SCr),urea nitrogen(BUN),urinary albumin excretion rate(UAER)of both groups were decreased,and those in the observation group were lower than those in the control group(P<0.05);there were no statistical significant difference in adverse reactions between the two groups(P>0.05).Conclusions Zhaqi jiangtang decoction combined with catgut implantation at acupoint is beneficial to relieve symptoms,improve indexes of blood glucose and renal function in the treatment of DN,it will not increase adverse reactions,with higher safety.
