摘 要:目的基于新型冠状病毒(SARS-CoV-2)在56℃加热30 min的灭活条件,探讨该条件下对于血清酶学项目检测结果的影响。方法采用完全随机化原则选取2021年1月于连云港市第二人民医院体检的健康人群50例作为研究对象,其中男性28例、女性22例,年龄(43.2±9.5)岁。基于配对设计的实验应用AU5800全自动生化分析仪分别检测灭活前和灭活后血清中淀粉酶(amylase,AMY)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、碱性磷酸酶(alkaline phosphatase,ALP)、γ-谷氨酰转肽酶(γ-glutamyl transpeptidase,GGT)、乳酸脱氢酶(lactate dehydrogenation enzyme,LDH)、腺苷脱氨酶(adenosine deaminase,ADA)和α-L-岩藻糖苷酶(α-L-fucosidase,AFU)等8个酶学项目,分别比较其灭活前后检测结果的差异,并采用Bland-Altman图对检测结果进行一致性分析。结果与灭活前的结果相比,灭活后ALT、AST、ALP、GGT、LDH、ADA、AFU的检测结果显著降低[ALT:19.00(12.75,22.60)U/L比14.50(10.75,20.25)U/L,Z=-5.978,P<0.05;AST:20.00(18.00,23.25)U/L比19.00(17.00,23.00)U/L,Z=-4.286,P<0.05;ALP:(85.68±36.11)U/L比(49.34±30.17)U/L,t=6.853,P<0.05;GGT:(22.14±10.20)U/L比(21.38±10.14)U/L,t=3.817,P<0.05;LDH:(154.40±24.34)U/L比(147.80±23.07)U/L,t=3.786,P<0.05;ADA:(8.61±2.22)U/L比(6.58±2.59)U/L,t=8.117,P<0.05;AFU:(25.64±6.29)U/L比(13.84±8.38)U/L,t=10.200,P<0.05],且Bland-Altman图分析显示上述酶学项目灭活前后结果不具有一致性。而血清AMY的检测结果在灭活前与灭活后差异无统计学意义[66.00(58.75,87.00)U/L比67.00(58.75,87.25)U/L,Z=-0.725,P=0.4687]。结论56℃加热30 min的条件对部分血清酶学项目的测定结果具有显著影响。因此,实验室使用该条件处理临床样本时,需采取必要的措施以校正检测结果。Objective Based on the inactivation condition of SARS-CoV-2 heated at 56℃ for 30 minutes,to explore the influence of this condition on the test results of serum enzymes.Methods Applying the principle of complete randomization,50 healthy people who received physical examination at Lianyungang Second People's Hospital in January 2021 was selected as the research objects,including 28 males and 22 females,with an age of(43.2±9.5)years old.Based on paired design,AU5800 automatic biochemical analyzer was used to detect the serum levels of amylase(AMY),alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),γ-glutamyl transpeptidase(GGT),lactate dehydrogenase(LDH),adenosine deaminase(ADA),andα-L-fucosidase(AFU)before and after the inactivation,and the detection results before and after the inactivation were compared.The Bland-Altman chart was used to analyze the results'consistency.Results Compared with the results before the inactivation,the results of serum ALT,AST,ALP,GGT,LDH,ADA,and AFU after the inactivation were significantly decreased[ALT:19.00(12.75,22.60)U/L vs.14.50(10.75,20.25)U/L,Z=-5.978,P<0.05;AST:20.00(18.00,23.25)U/L vs.19.00(17.00,23.00)U/L,Z=-4.286,P<0.05;ALP:(85.68±36.11)U/L vs.(49.34±30.17)U/L,t=6.853,P<0.05;GGT:(22.14±10.20)U/L vs.(21.38±10.14)U/L,t=3.817,P<0.05;LDH:(154.40±24.34)U/L vs.(147.80±23.07)U/L,t=3.786,P<0.05;ADA:(8.61±2.22)U/L vs.(6.58±2.59)U/L,t=8.117,P<0.05;AFU:(25.64±6.29)U/L vs.(13.84±8.38)U/L,t=10.200,P<0.05],and the Bland Altman chart analysis showed that the results before and after inactivation were not consistent.However,therewas no statistical difference between the detection results of serumamylase(AMY)before and after the inactivation[66.00(58.75,87.00)U/L vs.67.00(58.75,87.25)U/L,Z=-0.725,P=0.4687].Conclusions The condition of heating at 56℃for 30 minutes has a significant impact on the detection results of some serum enzymes.Therefore,when the laboratory uses this condition to process clinical samples,it needs to take necessar
