腹主动脉球囊置入术应用于凶险性前置胎盘伴植入的疗效分析

摘　　要：目的探讨腹主动脉球囊应用于凶险性前置胎盘的治疗效果以及对母儿安全性的影响。方法选取2017年1月至2020年12月于山东省青岛市市立医院终止妊娠的凶险性前置胎盘合并植入产妇88例为研究对象,根据术前是否置入腹主动脉球囊,分为球囊组36例和非球囊组52例。比较两组病例的一般资料、术中指标(手术时间、术中出血量、输血量等)、术后并发症(产褥感染、动静脉血栓等)及新生儿结局(出生体重、1min Apgar评分、5min Apgar评分、转入新生儿重症监护室例数)。结果球囊组的治疗费用和抗生素使用时间高于非球囊组,手术时间、术中出血量和输血量均少于非球囊组,差异有显著性(P<0.05)。新生儿结局比较差异均无显著性(P>0.05)。结论腹主动脉球囊置入术用于凶险性前置胎盘伴植入的剖宫产患者可有效缩短手术时间,减少术中出血量和输血量,提高母儿安全。Objective To investigate the practicability of preoperative abdominal aortic balloon occlusion for invasive placenta praevia and the effect on maternal and fetal safety.Method A total 88 pregnant women who were diagnosed as invasive placenta praevia,should be terminated the pregnancy from January,2017 to December,2020 in the Qingdao Municipal Hospital.36 women were preoperatively implanted an abdominal aortic balloon before the cesarean and 52 women didn’t.The operation length,operative indicators,postoperative complications,use of antibiotic,length of stay,treatment cost and neonatal information were observed.Result The cost of treatment and duration of antibiotic usage in balloon group were higher than those in non-balloon group(P<0.05),the time of operation,the hemorrhage volume and the transfusion volume were all lower than those in the non-balloon group(P<0.05).There were no significant differences in neonatal birth indexes(P>0.05).Conclusion Aortic balloon implantation can effectively shorten the operation time,reduce the amount of blood loss and blood transfusion during the operation,and improve the safety of mothers and children.
