摘　　要：通过在区域糖尿病患者的管理中引入全结构化的智慧病历系统,探讨如何在临床科研一体化的基础上,构建以糖尿病的防、治、康联动机制为手段的闭环管理模式,从而有效提高区域范围内的糖尿病患者健康管理水平。以智慧病历为核心,在专科、社区、患者之间搭建信息桥梁,充分发挥三级医院的专科指导意义、电子病历数据共享作用、医联体间分级诊疗及双向转诊的服务价值,进一步提高患者自我健康管理意识,以此为区域糖尿病管理的规范性、科学性、实用性提供意见与思路,最终能够建立院前预防、院中治疗、院后康复的全程管理模式。By introducing a fully structured intelligent medical record system into the management of regional diabetes patients,this paper discusses how to build a closed-loop management mode based on the linkage mechanism of diabetes prevention,treatment and health on the basis of the integration of clinical scientific research,so as to effectively improve the health management level of regional diabetes patients.Medical record as the core,set up information bridge among specialists,patients and community,give full play to the tertiary hospital of specialized subject guide,electronic medical record data sharing function,and two-way referral between couplet of grading diagnosis and treatment of service value,further improve patients self health management consciousness,to provide advices and ideas to the normative,scientific,practical of the regional diabetes management,it can eventually establish the whole hospital management mode of pre-hospital prevention,the treatment in the hospital and the rehabilitation after treatment.
