鼻内窥镜下低温等离子辅助切除腺样体及扁桃体治疗儿童鼾症的临床疗效

摘　　要：目的讨论鼻内窥镜下低温等离子辅助切除腺样体及扁桃体治疗儿童鼾症的效果。方法52例鼾症患儿,以掷硬币法分为治疗组和对照组,每组26例。对照组采取常规治疗,治疗组采取鼻内窥镜下低温等离子辅助切除腺样体及扁桃体治疗。比较两组手术耗时、术中出血量、疼痛持续时间、最低动脉血氧饱和度、睡眠呼吸暂停通气指数、睡眠质量评分、并发症发生率。结果治疗组手术耗时(26.13±4.06)min、疼痛持续时间(3.21±0.29)d均短于对照组的(40.85±6.32)min、(5.79±0.46)d,术中出血量(27.16±6.05)ml少于对照组的(55.69±9.37)ml,差异均具有统计学意义(P<0.05)。治疗后,治疗组患儿最低动脉血氧饱和度(94.35±11.28)%、睡眠呼吸暂停通气指数(2.36±0.43)次/h、睡眠质量评分(5.32±0.54)分均优于对照组的(86.32±9.22)%、(4.57±0.93)次/h、(9.65±0.95)分,差异均具有统计学意义(P<0.05)。治疗组并发症发生率3.85%低于对照组的26.92%,差异具有统计学意义(P<0.05)。结论鼾症患儿采取鼻内窥镜下低温等离子辅助切除腺样体及扁桃体治疗可显著提高临床效果,且治疗安全性高,有临床应用价值。Objective To discuss the effect of low-temperature plasma assisted adenoidectomy and tonsillectomy under nasal endoscope in the treatment of snoring in children.Methods A total of 52 children with snoring were divided into treatment group and control group by coin tossing method,with 26 cases in each group.The control group received conventional treatment,while the treatment group received low-temperature plasma assisted adenoidectomy and tonsillectomy under nasal endoscope.Both groups were compared in terms of operation time,intraoperative blood loss,pain duration,minimum arterial oxygen saturation,sleep apnea ventilation index,sleep quality score,and complication rate.Results The operation time(26.13±4.06)min and pain duration(3.21±0.29)d in the treatment group were shorter than(40.85±6.32)min and(5.79±0.46)d in the control group,and the intraoperative blood loss(27.16±6.05)ml was less than(55.69±9.37)ml in the control group.All the differences were statistically significant(P<0.05).After treatment,the minimum arterial oxygen saturation(94.35±11.28)%,sleep apnea ventilation index(2.36±0.43)/h,and sleep quality score(5.32±0.54)points of patients in the treatment group were better than(86.32±9.22)%,(4.57±0.93)/h,and(9.65±0.95)points in the control group,and the differences were statistically significant(P<0.05).The complication rate 3.85%of the treatment group was lower than 26.92%of the control group,and the difference was statistically significant(P<0.05).Conclusion Low temperature plasma assisted adenoidectomy and tonsillectomy under nasal endoscope can significantly improve the clinical effect of snoring children,and the treatment is safe and valuable.
