厄贝沙坦氢氯噻嗪片联合美托洛尔治疗慢性充血性心衰的效果观察

摘　　要：目的探讨厄贝沙坦氢氯噻嗪片联合美托洛尔治疗慢性充血性心力衰竭(心衰)的临床疗效。方法78例慢性充血性心衰患者,随机分为观察组和对照组,各39例。对照组采取厄贝沙坦氢氯噻嗪片治疗,观察组采取厄贝沙坦氢氯噻嗪片联合美托洛尔治疗。比较两组临床疗效及治疗前后心功能指标、氨基末端脑钠肽前体(NT-ProBNP)及6 min步行距离。结果观察组总有效率94.9%高于对照组的79.5%,差异有统计学意义(P<0.05)。治疗前,两组左室射血分数(LVEF)、左室舒张末期内径(LVEDd)、左室收缩末期内径(LVESd)、NT-ProBNP及6 min步行距离比较,差异无统计学意义(P>0.05);治疗后,两组LVEF、LVEDd、LVESd、NT-ProBNP及6 min步行距离均明显优于治疗前,且观察组LVEF(48.9±5.1)%、LVEDd(50.2±3.3)mm、LVESd(33.6±3.1)mm、NT-ProBNP(2315.9±375.2)pg/ml及6 min步行距离(441.6±75.2)m均优于对照组的(38.3±4.1)%、(58.3±3.9)mm、(42.5±3.4)mm、(3752.4±326.0)pg/ml、(360.7±82.5)m,差异有统计学意义(P<0.05)。结论厄贝沙坦氢氯噻嗪片联合美托洛尔治疗慢性充血性心衰临床疗效显著,可有效改善心功能,提高运动耐量,值得推广。Objective To discuss the clinical efficacy of irbesartan and hydrochlorothiazide tablets combined with metoprolol in the treatment of chronic congestive heart failure.Methods A total of 78 patients with chronic congestive heart failure were randomly divided into control group and observation group,with 39 cases in each group.The control group was treated with irbesartan and hydrochlorothiazide tablets,and the observation group was treated with irbesartan and hydrochlorothiazide tablets in combiation with metoprolol.Both groups were compared in terms of clinical efficacy,cardiac function indexes,N-terminal pro-brain natriuretic peptide(NT-ProBNP)and 6-min walking distance before and after treatment.Results The total effective rate 94.9%of the observation group was higher than 79.5%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no statistically significant differences in left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDd),left ventricular end-systolic diameter(LVESd),NT-ProBNP and 6-min walk distance between the two groups(P>0.05).After treatment,LVEF,LVEDd,LVESd,NT-ProBNP and 6-min walking distance in both groups were significantly better than those before treatment;LVEF(48.9±5.1)%,LVEDd(50.2±3.3)mm,LVESd(33.6±3.1)mm,NT-ProBNP(2315.9±375.2)pg/ml and 6-min walking distance(441.6±75.2)m in the observation group were better than(38.3±4.1)%,(58.3±3.9)mm,(42.5±3.4)mm,(3752.4±326.0)pg/ml,(360.7±82.5)m in the control group;all the differences were statistically significant(P<0.05).Conclusion Combination of irbesartan and hydrochlorothiazide tablets and metoprolol has significant clinical efficacy on the treatment of chronic congestive heart failure,and it can effectively heart function and exercise tolerance,and is worthy of promotion.
