伤科黄水外敷治疗急性软组织损伤的疗效观察

摘　　要：目的观察伤科黄水外敷治疗四肢急性软组织损伤的疗效并探讨其作用机制。方法156例患者随机分为对照组与观察组各78例。对照组予以常规治疗,观察组予以常规治疗联合伤科黄水外敷。比较两组的临床疗效,治疗前后的视觉模拟疼痛评分(VAS)、压痛评分、功能障碍评分、肿胀评分和血清肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素-6(IL-6),肿胀、功能障碍和疼痛缓解时间变化情况。结果观察组总有效率97.44%高于对照组的88.46%(P<0.05)。两组治疗后VAS、肿胀周径差值均较治疗前降低,且治疗后观察组VAS、肿胀周径差值均低于对照组(均P<0.05)。治疗后,两组压痛、瘀斑、肿胀、功能障碍评分均较治疗前降低,且治疗后观察组压痛、瘀斑、肿胀、功能障碍评分均低于对照组(均P<0.05)。治疗后,观察组疼痛、肿胀、功能障碍缓解时间均短于对照组(P<0.05)。治疗后,两组血清TNF-α、CRP、IL-6均较治疗前降低,且治疗后观察组均低于对照组(均P<0.05)。结论伤科黄水外敷治疗四肢急性软组织损伤可以缓解疼痛、肿胀,促进康复,抑制炎症。Objective:To observe the effects of external application of Shangke Huangshui on acute soft tissue injury of extremities.Methods:A total of 156 patients with acute soft tissue injury of extremities were randomly divided into the control group and the observation group,78 cases in each group.The control group received conventional treatment,and the observation group received conventional treatment combined with external application of Shangke Huangshui.The clinical efficacy of the two groups,visual analog pain score(VAS),tenderness score,dysfunction score,swelling score,serum tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),and interleukin-6(IL-6)before and after treatment.Swelling,dysfunction and pain relief time were compared.Results:After treatment,the total effective rate of the observation group was significantly higher than that of the control group(P<0.05);after treatment,the scores of VAS,tenderness,dysfunction,and swelling of the two groups were lower than those before treatment(P<0.05),and the scores of VAS,tenderness,dysfunction,and swelling in the observation group were significantly lower than those in the control group(P<0.05);after treatment,the levels of TNF-α,CRP,and IL-6 in the two groups were lower than before treatment(P<0.05),and the levels of TNF-α,CRP,IL-6 in the observation group were significantly lower than those in the control group(P<0.05);after treatment,the swelling,dysfunction and pain relief time in the observation group were significantly shorter than those in the control group(P<0.05).Conclusion:External application of Shangke Huangshui for acute soft tissue injury of extremities can relieve pain and swelling,promote recovery,and inhibit inflammation.
