真武汤合葶苈大枣泻肺汤治疗急性心力衰竭患者的临床观察

摘　　要：目的观察真武汤合葶苈大枣泻肺汤治疗急性心力衰竭患者的临床疗效。方法选择我院从2019年7月至2020年7月收治的60例急性心力衰竭(阳虚水泛证)患者随机分为两组,对照组给予西医常规治疗,观察组在西医治疗方案基础上加用真武汤合葶苈大枣泻肺汤,观察两组治疗1周前后CA125、N末端B型利钠肽原(NT-proBNP)、左心室射血分数(LVEF)、心功能分级。结果观察组总有效率86.67%显著高于对照组70.00%(P<0.05);两组气短、心悸、乏力、浮肿主要中医证候积分较前显著改善(P<0.05),观察组显著优于对照组(P<0.05);治疗后,两组CA125、NT-proBNP较治疗前显著降低(P<0.05),LVEF较前显著升高(P<0.05),观察组改善优于对照组(P<0.05);对照组不良反应为33.33%,显著高于观察组的13.33%(P<0.05)。结论真武汤合葶苈大枣泻肺汤对急性心力衰竭患者临床疗效好,能降低CA125表达、有效改善患者心功能,安全性高,不良反应少。Objective:To explore the effect of Zhenwu Decoction and Tingli Dazao Xiefei Decoction on patients with acute heart failure.Methods:A total of 60 patients with acute heart failure(syndrome of water overflowing due to yang deficiency)in our hospital from July 2019 to July 2020 were selected and randomly divided into two groups.The control group was given conventional Western medicine treatment,and the observation group was added with Zhenwu Decoction and Tingli Dazao Xiefei Decoction on the basis of the Western medicine treatment.The changes of CA125,N-terminal pro-B-type natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF),and cardiac function grading in the two groups before and after 1 week of treatment were observed.Results:The total effective rate of 86.67%in the observation group was significantly higher than 70.00%in the control group(P<0.05);the main TCM syndrome index indicators of shortness of breath,palpitations,fatigue,and edema in the two groups were significantly improved compared with before treatment(P<0.05),and the observation group was significantly better than the control group(P<0.05);after treatment,CA125 and NT-proBNP of the two groups were significantly lower than before treatment(P<0.05),LVEF was significantly higher than before treatment(P<0.05),and the observation group was significantly improved compared with the control group(P<0.05).The adverse reactions of the control group accounted for 33.33%,which was significantly higher than 13.33%of the observation group(P<0.05).Conclusion:Zhenwu Decoction combined with Tingli Dazao Xiefei Decoction has a good clinical effect on patients with acute heart failure.It reduces the expression of CA125,and can effectively improve the cardiac function of patients.It has high safety and few adverse reactions.
