摘  要:目的了解聊城市人间布病的时空聚集特点和职业人群发病危险因素,为制定科学防控措施提供依据。方法描述性流行病学方法分析2018年全市人间布病疫情资料流行病学分布;使用ArcGis 10.2、SaTScan v 9.6进行空间分析及时空聚集性分析;使用SPSS 24.0对布病试管凝集实验(SAT)重点筛查数据进行单因素χ2检验及Logistic回归分析。结果2018年聊城市人间布病发病主要集中在3~7月;男女比为3.17∶1;农民为主;从事羊牛饲养、贩卖、屠宰、皮毛加工职业人群为发病的重点人群,该人群集中分布的6个地区为发病重点地区,与时空聚集性分析的结果高度一致。试管凝集实验(SAT)检测阳性率比较,回族高于汉族,小学以下学历感染率高,从事牛羊屠宰工作,接触过孕羊牛流产、有割伤经历、无预防措施、庭院散养人群的感染率高。洗手、穿隔离衣、戴手套、实行定点屠宰等职业人群的的感染率低,会鉴别病畜、避免接触流产的孕羊牛、食用炖羊肉而是预防布病的保护因素。结论聊城市人间布病具有明显的时空聚集性;接触孕羊牛流产、散养于庭院、食用未煮熟羊肉、工作中无防护措施、处理过程中受伤可增加布病发病风险。Objective To understand the spatial and temporal clustering characteristics of brucellosis and risk factors of occupational population in Liaocheng city,so as to provide basis for formulating scientific prevention and control measures.Methods Descriptive epidemiological analysis was used to describe the epidemiological distribution of human brucellosis epidemic data in 2018.ArcGis 10.2 and SaTScan v 9.6 were used for spatial analysis and spatial-temporal aggregation analysis.SPSS 24.0 was used for singleχ^(2) analysis and logistic regression analysis of brucellosis key screening data.Results The incidence of brucellosis in Liaocheng city in 2018 was mainly from March to July;the ratio of male to female was 3.17∶1;Farmers were the main group;sheep and cattle breeding,trafficking,slaughtering and fur processing were the key groups of the disease.The 6 areas where the population was concentrated were the focus areas,and the results of time-space aggregation analysis were highly consistent.The infection rate of serological SAT and RBPT was higher in Hui than that in Han nationality.The infection rate of primary school and lower education level was higher.Those who were engaged in cattle and sheep slaughter,contacted with pregnant sheep and cattle,had abortion,cut experience,no preventive measures,and backyard backyard had high infection rate.Hand washing,wearing isolation gown,wearing gloves,the implementation of fixed-point slaughter and other occupational groups of low infection rate,will identify sick animals,avoid contact with abortion of pregnant sheep and cattle,eating stewed mutton but the protection factors to prevent brucellosis.Conclusion Brucellosis has obvious spatial and temporal aggregation in Liaocheng city.The risk of brucellosis could be increased by abortion after contact with pregnant sheep and cattle,loose feeding in the courtyard,eating undercooked mutton,no protective measures at work,and injury during treatment.
