益肾活血接骨汤辅助复位固定治疗肾虚血瘀型老年性桡骨远端骨折临床研究

摘　　要：目的:观察益肾活血接骨汤辅助复位固定治疗肾虚血瘀型老年性桡骨远端骨折的临床疗效。方法:将60例肾虚血瘀型老年性桡骨远端骨折患者随机分为治疗组与对照组各30例。对照组采用复位固定治疗,治疗组采用益肾活血接骨汤辅助复位固定治疗。观察2组治疗前后视觉模拟评分法(VAS)评分、患侧肿胀程度、血小板计数(BPC)、凝血酶时间(TT)、凝血酶原时间(PT)及骨密度(BMD)、骨代谢水平,比较2组临床疗效及不良反应发生率。结果:治疗组总有效率为96.67%,高于对照组的76.67%(P<0.05)。与同组治疗前比较,治疗后2组VAS评分及肿胀程度均降低(P<0.05),BPC、TT、PT及BMD、骨源性碱性磷酸酶(BALP)、Ⅰ型前胶原羧基端前肽(PICP)、骨钙素(BGP)水平均升高(P<0.05);与对照组治疗后比较,治疗组治疗后VAS评分及肿胀程度均较低(P<0.05),BPC、TT、PT及BMD、BALP、PICP、BGP水平均较高(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:益肾活血接骨汤辅助复位固定治疗肾虚血瘀型老年性桡骨远端骨折患者疗效较好,可有效改善症状和凝血功能,降低疼痛,促进骨形成和关节恢复,安全可靠。Objective:To observe the clinical effect of Yishen Huoxue Jiegu tang assisting reduction and fixation for senile distal radius fracture of kidney deficiency and blood stasis type. Methods:A total of 60 cases of patients with senile distal radial fracture of kidney deficiency and blood stasis type were randomly divided into the treatment group and the control group, 30 cases in each group. The control group was treated with reduction and fixation, and the treatment group was additionally treated with Yishen Huoxue Jiegu tang based on the treatment of the control group. Before and after treatment,Visual Analogue Scale(VAS) scores, swelling degree of the affected side, platelet count(BPC), thrombin time(TT),prothrombin time(PT),and levels of bone mineral density(BMD) and bone metabolism in the two groups were observed,and the clinical effect and incidences of adverse reactions in the two groups were compared. Results:The total effective rate was96.67% in the treatment group,higher than that of 76.67% in the control group(P<0.05). After treatment,the VAS scores and swelling degrees in the two groups were decreased when compared with those before treatment(P<0.05),and BPC,TT,PT,and levels of BMD,bone alkaline phosphatase(BALP),carboxyterminal propeptide of type I procollagen(PICP) and bone gla protein(BGP) were increased(P<0.05). After treatment,the VAS score and swelling degree in the treatment group were lower than those in the control group(P<0.05),and BPC,TT,PT and levels of BMD,BALP,PICP and BGP were higher(P<0.05). There was no significant difference being found in the comparison of adverse reactions between the two groups(P>0.05). Conclusion: Yishen Huoxue Jiegu tang assisting reduction and fixation for senile distal radial fracture of kidney deficiency and blood stasis type has a good curative effect,which can effectively improve symptoms and coagulation function,reduce pain,and promote bone formation and joint recovery,which is safe and reliable.
