摘 要:目的分析金昌队列人群2011—2020年糖尿病的死亡率、疾病负担及变化趋势,评价该队列人群糖尿病的死亡特征及疾病负担情况,为糖尿病的防治提供基础数据。方法以金昌队列为平台,以基线48001人为研究对象,连续随访10 a,应用Joinpoint回归法计算粗死亡率、标化死亡率、疾病负担及年度变化百分比等指标。结果2011—2020年金昌队列人群糖尿病粗死亡率为27.16/10万,标化死亡率为15.04/10万,其中男性粗死亡率为35.86/10万,标化死亡率为17.74/10万;女性粗死亡率为12.92/10万,标化死亡率为6.41/10万,男性高于女性,变化趋势均无统计学意义(P>0.05)。70岁及以上人群糖尿病死亡率最高(156.26/10万),在总死亡人数中占比最高(88.24%)。糖尿病造成的潜在减寿年数为451.89 a,平均潜在减寿年数为10.51 a/人,潜在工作损失年数为212.50 a,平均潜在工作损失年数为9.24 a/人。结论金昌队列人群糖尿病死亡率相对较高,且集中在高年龄段人群,男性糖尿病死亡率与疾病负担较重,应重点关注并尽早给予相应的预防措施,降低死亡率。Objective To analyze the changing trend of diabetic mortality and disease burden in the Jinchang cohort from 2011 to 2020,evaluate the death characteristics and disease burden of diabetes in this cohort and provide basic data for the prevention and treatment of diabetes.Methods Using the Jinchang cohort as a platform,48001 people from the baseline were included,and a continuous follow-up was carried out for ten years.The Joinpoint regression method was used to calculate indicators such as crude mortality rate,age-stan-dardized mortality rate,disease burden and annual percent change and other indicators.Results From 2011 to 2020,the crude mortality rate of diabetes in the Jinchang cohort was 27.16/100000,and the age-standardized mortality rate 15.04/100000,of which the male crude mortality rate was 35.86/100000 and the age-standardized mortality rate 17.74/100000.The crude mortality rate for females was 12.92/100000,and the age-stan-dardized mortality rate 6.41/100000.Males were higher than females,and the changing trend was not statistically significant(P>0.05).Mortality from diabetes was the highest in people aged 70 and above(156.26/100000),accounting for the largest number of deaths(88.24%).Potential years of life lost caused by diabetes was 451.89,average potential years of life lost was 10.51,work years of potential life lost was 212.50,and aver-age work years of potential life lost was 9.24.Conclusion The mortality rate of diabetes in the Jinchang cohort was higher and was concentrated in the elderly.The mortality and disease burden of male diabetes was higher and heavier.Appropriate preventive measures are to be taken as soon as possible to reduce the mortality.
