摘　　要：目的评价"Angiographosome"理念指导下靶动脉开通术治疗Rutherford 5、6级缺血的临床效果。方法回顾性分析2018年6月至2019年6月单中心采用Angiographosome理念完成膝下靶血管重建术治疗的20例下肢缺血性溃疡患者临床资料。其中Rutherford 5级缺血16例,6级缺血4例。术后1、3、6个月随访观察创面愈合情况及截肢、死亡事件。结果 20例膝下靶血管开通术均获成功,无手术技术相关并发症发生。术后1、3、6个月创面愈合率分别为30%(6/20)、70%(14/20)、80%(16/20),其中Rutherford 5级缺血患者分别为37.5%(6/16)、87.5%(14/16)、100%(16/16);术后6个月Rutherford 6级缺血患者创面缩小明显,但未完全愈合。随访期所有患者无大截肢事件和死亡事件发生。结论Angiographosome理念指导下膝下靶血管开通术安全有效,尤其是对Rutherford 5级缺血患者,可取得良好的创面愈合率和保肢率。Objective To evaluate the clinical effect of angiographosome-directed revascularization of target artery below knee joint in treating Rutherford class 5 and 6 ischemia. Methods The clinical data of 20 patients with ischemic ulcer of lower extremity, who underwent angiographosome-directed revascularization of target artery below knee joint at a single center during the period from June 2018 to June 2019, were retrospectively analyzed. Of the 20 patients, 16 had Rutherford class 5 ischemia and 4 had Rutherford class 6 ischemia. Postoperative one-, 3-and 6-month follow-up was conducted to record the wound-healing condition, the events of amputation and death. Results Successful revascularization of target artery below knee joint was accomplished in all the 20 patients, no procedure-related complications occurred. The postoperative one-, 3-and 6-month wound-healing rates were 30%(6/20), 70%(14/20) and 80%(16/20)respectively, which in the patients with Rutherford class 5 ischemia were 37.5%(6/16), 87.5%(14/16), and100%(16/16) respectively. In the patients with Rutherford class 6 ischemia, the wound area was obviously reduced in 6 months after treatment although it was not completely healed. During the follow-up period no major events such as amputation or death occurred in all patients. Conclusion For the treatment of ischemic ulcer of lower extremity, especially for patients with Rutherford class 5 ischemia, angiographosome-directed revascularization of target artery below knee joint is clinically safe and effective, it has excellent wound-healing rate and limb salvage rate.(J Intervent Radiol, 2021, 30: 994-997)
