摘 要:目的:通过检测NLRP3/IL-1β信号通路在高尿酸性肾病患者体内的水平,探讨其在高尿酸性肾病中的作用。方法:选择2018年9月-2020年9月本院收治的高尿酸血症患者42例和高尿酸性肾病患者26例,选取同期体检的健康者35名作为健康对照组。分离研究对象外周血单核细胞,采用实时荧光定量PCR法检测NLRP3和caspase-1 mRNA水平,酶联免疫吸附试验法(ELISA)检测血浆IL-1β、TNF-α、IL-6、IL-18的水平,比较各组上述指标。结果:高尿酸血症组胱抑素C、尿素氮、尿酸水平均高于健康对照组,差异均有统计学意义(P<0.05)。高尿酸性肾病组胱抑素C、尿素氮及血肌酐水平均明显高于健康对照组及高尿酸血症组,差异均有统计学意义(P<0.05)。三组BMI、血压、血糖、三酰甘油、胆固醇、低密度脂蛋白、高密度脂蛋白比较,差异均无统计学意义(P>0.05)。高尿酸血症组和高尿酸性肾病组NLRP3、caspase-1 mRNA、IL-1β、TNF-α、IL-6、IL-18水平均高于健康对照组,高尿酸性肾病组NLRP3、caspase-1 mRNA、IL-1β、TNF-α、IL-6、IL-18水平均高于高尿酸血症组,差异均有统计学意义(P<0.05)。结论:NLRP3/IL-1β信号通路和TNF-α、IL-6、IL-18等炎性细胞因子参与了高尿酸性肾病的发生和发展。Objective:To detect the level of NLRP3/IL-1β signaling pathway in patients with hyperuric acid nephropathy,to explore its role in hyperuric acid nephropathy.Method:A total of 42 patients with hyperuricemia and 26 patients with hyperuricemic nephropathy admitted to our hospital from September 2018 to September 2020 were selected,and 35 healthy people who underwent physical examination during the same period were selected as the healthy control group.Peripheral blood monocytes were isolated,the mRNA levels of NLRP3 and caspase-1 were detected by real-time fluorescence quantitative PCR,plasma levels of IL-1β,TNF-α,IL-6 and IL-18 were detected by enzyme-linked immunosorbent assay(ELISA),the above indexes of each group were compared.Result:The levels of cystatin C,urea nitrogen and uric acid in hyperuricemia group were higher than those in healthy control group,the differences were statistically significant(P<0.05).The levels of cystatin C,urea nitrogen and serum creatinine in hyperuricemia nephropathy group were significantly higher than those in healthy control group and hyperuricemia group,the differences were statistically significant(P<0.05).Comparison of BMI,blood pressure,blood glucose,triglycerides,cholesterol,low density lipoprotein and high density lipoprotein among the three groups,there were no significant differences(P>0.05).The levels of NLRP3,caspase-1 mRNA,IL-1β,TNF-α,IL-6 and IL-18 in hyperuricemia group and hyperuricemia nephropathy group were higher than those in healthy control group,the levels of NLRP3,caspase-1 mRNA,IL-1β,TNF-α,IL-6 and IL-18 in hyperuricemia nephropathy group were higher than those in hyperuricemia group,the differences were statistically significant(P<0.05).Conclusion:NLRP3/IL-1β signaling pathway and inflammatory cytokines such as TNF-α,IL-6 and IL-18 are involved in the occurrence and development of hyperuric acid nephropathy.
