摘　　要：目的:探讨VSD负压引流在经大腿皮下通路腔镜下外阴癌根治术中的临床护理应用,以期为临床医护工作提供参考。方法:选择2019年1月-2021年1月本院收治的82例经大腿皮下通路腔镜下外阴癌根治术患者。采用随机数字表法将患者分为对照组和观察组,每组41例。对照组患者采用传统引流管,观察组患者采用VSD负压引流装置。比较两组治疗前后焦虑和抑郁情况、生活质量,比较两组干预满意度及并发症发生情况。结果:治疗后,两组SDS、SAS评分均低于治疗前,且观察组均低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组患者躯体功能、情绪功能、认知功能和整体生活质量评分均高于治疗前,且观察组均高于对照组,差异均有统计学意义(P<0.05)。观察组干预总满意度为97.56%,高于对照组的78.05%,差异有统计学意义(P<0.05)。观察组并发症发生率为9.76%,低于对照组的34.15%,差异有统计学意义(P<0.05)。结论:VSD负压引流在经大腿皮下通路腔镜下外阴癌根治术中价值较高,可改善患者生活质量,减轻焦虑和抑郁情绪,减少并发症,值得临床医护借鉴。Objective:To explore the clinical nursing application research of VSD negative pressure drainage in the laparoscopic radical resection of vulvar cancer through the subcutaneous passage of the thigh,in order to provide a reference for clinical medical care.Method:A total of 82 patients admitted to our hospital from January 2019 to January 2021 were selected for radical resection of vulvar cancer through subcutaneous access of the thigh under endoscopy.Patients were divided into control group and observation group by random number table method,41 cases in each group.Traditional drainage tube was used in control group,VSD negative pressure drainage device was used in observation group.The anxiety,depression and quality of life of the two groups were compared before and after treatment,the intervention satisfaction and complications were compared between the two groups.Result:After treatment,the SDS and SAS scores of the two groups were lower than those before treatment,and those of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,the scores of physical function,emotional function,cognitive function and overall quality of life in the two groups were higher than those before treatment,and those in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).The total satisfaction rate of intervention in the observation group was 97.56%,higher than 78.05%in the control group,the difference was statistically significant(P<0.05).The complication rate in the observation group was 9.76%,lower than 34.15%in the control group,the difference was statistically significant(P<0.05).Conclusion:VSD negative pressure drainage has good value in endoscopic radical resection of vulvar cancer through thigh subcutaneous pathway,it can improve patients’quality of life,reduce anxiety and depression,and reduce complications,it is worthy of reference for clinical doctors and nurses.
