摘 要:目的:探究瘦素基因多态性与复发性自然流产(RSA)风险的潜在关联性。方法:选取2016年5月-2019年4月来本院就诊的138例复发性流产的患者作为RSA组,选择同期于本院产检的142例正常妊娠妇女为对照组。所有入组研究对象禁饮食,取5 mL外周静脉血,利用高效液相色谱(DHPLC)法测定所有基因型,采用ELISA法测定血清瘦素水平。结果:在Leptin rs7799039 G/A的基因型的多态性中,RSA组的GA+AA型、AA型频率以及A等位基因型频率均高于对照组,GG型频率低于对照组(P<0.05)。在Leptin rs7799039 G/A基因多态性中,RSA组中含有A等位基因和不含非A等位基因的患者,两者的临床表现不同,携带GG基因型患者的严重妊娠反应、妊娠期流产时间(≥3个月)、流产≥3次、与下一次妊娠时间间隔(≥6个月)的发生率均高于GA+AA基因型,差异均有统计学意义(P<0.05)。在对照组中,GG基因型妊娠者的血浆瘦素水平为(4.87±0.36)ng/mL,虽略低于GA+AA基因型的(5.01±0.39)ng/mL,但差异无统计学意义(P=0.101);在RSA组中,携带GG基因型RSA患者的瘦素水平为(6.38±0.45)ng/mL,低于基因型为GA+AA型的(6.77±0.56)ng/mL,差异有统计学意义(P=0.026)。结论:Leptin rs7799039 G/A的A基因突变可以增加RSA的发病风险。Objective:To investigate the possible associations of Leptin polymorphisms with the risk of recurrent spontaneous abortion(RSA).Method:A total of 138 patients with recurrent abortion who visited our hospital from May 2016 to April 2019 were selected as RSA group,and 142 normal pregnant women who underwent obstetric examination in our hospital during the same period were selected as control group.All subjects were forbidden to eat,5 mL peripheral venous blood was taken,all genotypes were determined by High performance liquid chromatography(DHPLC),and serum leptin level was determined by ELISA.Result:In the polymorphism of Leptin RS7799039 G/A genotype,the frequency of GA+AA type,AA type and A allele in RSA group were higher than those in control group,GG type was lower than that in the control group(P<0.05).In Leptin RS7799039 G/A gene polymorphism,patients with A allele and patients without non-A allele in RSA group have different clinical manifestations,The incidence of severe pregnancy reaction,abortion time during pregnancy(≥3 months),abortion≥3 times and interval between pregnancy and next pregnancy(≥6 months)in patients with GG genotype were higher than those with GA+AA genotype,the differences were statistically significant(P<0.05).In the control group,the plasma leptin level of GG genotype pregnancy was(4.87±0.36)ng/mL,although slightly lower than(5.01±0.39)ng/mL of GA+AA genotype,but the difference was not statistically significant(P=0.101);in RSA group,the leptin level of patients with GG genotype RSA was(6.38±0.45)ng/mL,lower than(6.77±0.56)ng/mL of patients with GA+AA genotype,the difference was statistically significant(P=0.026).Conclusion:Leptin rs7799039 G/A mutation of A gene can increase the risk of RSA.
