玉屏风颗粒联合甲泼尼龙琥珀酸钠对急性呼吸窘迫综合征患者血气指标、炎性因子及免疫功能的影响

摘　　要：目的:探讨玉屏风颗粒联合甲泼尼龙琥珀酸钠对急性呼吸窘迫综合征(ARDS)患者血气指标、炎性因子及免疫功能的影响。方法:采用前瞻性的研究方法,按住院序号将2016年9月-2018年9月在本院治疗的96例ARDS患者分为观察组和对照组,各48例。两组均给予常规治疗,对照组在常规治疗的基础上给予甲泼尼龙琥珀酸钠治疗,观察组在对照组基础上辅助给予玉屏风颗粒治疗,两组均连续用药7 d。比较两组治疗前后血气指标、炎性因子水平、细胞免疫功能、急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分及不良反应。结果:与治疗前比较,两组治疗后PaO_(2)、SaO_(2)、OI均升高,血清IL-6、TNF-α、IL-17A水平均降低,且观察组变化较对照组更明显,差异均有统计学意义(P<0.05)。与治疗前比较,两组治疗后CD8^(+)较治疗前均降低,且观察组低于对照组,两组治疗后CD4^(+)、CD4^(+)/CD8^(+)均升高,观察组均高于对照组,差异均有统计学意义(P<0.05)。与治疗前比较,两组治疗后APACHEⅡ评分均降低,且观察组低于对照组,差异均有统计学意义(P<0.05)。观察组不良反应发生率为4.17%,与对照组的6.25%比较,差异无统计学意义(χ^(2)=0.223,P=0.629)。结论:在常规治疗基础上给予玉屏风颗粒联合甲泼尼龙琥珀酸钠可更有效地减轻ARDS患者机体炎症反应,改善患者血气指标,减轻ARDS病情严重程度,同时还可明显增强机体细胞免疫功能,且安全性较高。Objective:To explore the influence of Yupingfeng Particles combined with Methylprednisolone Sodium Succinate on blood gas indicators,inflammatory factors and immune function in patients with acute respiratory distress syndrome(ARDS).Method:Using prospective research method,96 ARDS patients treated in our hospital from September 2016 to September 2018 were divided into the observation group and the control group according to the serial number,48 cases in each group.Both groups were given conventional treatment,the control group was treated with Methylprednisolone Sodium Succinate on the basis of conventional treatment,and the observation group was treated with Yupingfeng Particles on the basis of the control group,two groups were treatment continuously for 7 d.The blood gas indicators,levels of inflammatory factors,cellular immune function,acute physiology,chronic health status scoring systemⅡ(APACHEⅡ)scores before and after treatment and the incidence of adverse reactions were compared between two groups.Result:Compared with before treatment,the PaO_(2),SaO_(2) and OI after treatment of two groups increased,the levels of serum IL-6,TNF-α,IL-17A decreased,the changes of the observation group were more obvious,the differences were statistically significant(P<0.05).Compared with before treatment,the CD8^(+)after treatment in both groups decreased,the observation group was lower than that of the control group,the CD4^(+)and CD4^(+)/CD8^(+)increased,the observation group were higher than those the control group,the differences were statistically significant(P<0.05).Compared with before treatment,the scores of APACHEⅡafter treatment in both groups decreased,the observation group was lower than that of the control group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 4.17%,compared with 6.25%in the control group,there was no statistically significant difference(χ^(2)=0.223,P=0.629).Conclusion:Yupingfeng Particles combined with Methylprednisol
